STATEMENT OF WORK (SOW) Title: Comprehensive Service Contract on Illumina MiSeq System Background: MiSeq desktop sequencer is a highly technical piece of equipment manufactured by Illumina Inc. MiSeq requires routine maintenance and service by highly trained and manufacturer-certified personnel. The MiSeq desktop sequencer allows the users to access many different focused applications, such as targeted gene sequencing, metagenomics, small genome sequencing, targeted gene expression, amplicon sequencing, and HLA typing. MiSeq reagents enable up to 15 Gb of output with 25 M sequencing reads and 2x300 bp read lengths. This allows the users to obtain small genome sequencing and assembly, and enables detection of target variants with unmatched accuracy. In addition, MiSeq enables the users to process multiple samples simultaneously in a timely manner (less then 24hrs), generating more reads per run than any previous versions of desktop sequencers. Finally, using MiSeq, the users are able to: (1) access the world's most widely-published and accurate sequencing chemistry, backed by > 4,800 peer-reviewed publications. (2) multiplex up to 384 samples per run for greater efficiency, and (3) obtain accurate bi-directional amplicon sequencing. Scope of Work: The scope of the work requires a comprehensive service on Illumina MiSeq S# M02287. Service includes labor, travel and replacement of all parts; component inspections; subsystem maintenance; system requalification; a certification document as well as preventive maintenance (PM) visits. System Health Check is required before the initiation of the contract. Specific Tasks: The tasks include preventive maintenance (PM) visit (1 per year) and repairs when (and if) necessary during the entire length of the contract. All tasks include cost of labor, travel and replacement of all parts; component Inspections; subsystem maintenance; system requalification; certification documents, reagent replacement upon hardware (HW) failures; remote technical support; 3 business day average on site response; HW/ software (SW) updates; on site applications support. Task One Enterprise Management Controls: Contractor shall schedule all PM visits in advance with the COR or COR s designee. Emergency repairs shall be responded to Monday to Friday 8am -4:30pm and will continue until repair is completed. Contract Management: Preventative maintenance shall be no less than one visit per year and all service and repairs as need. Task Two: Provide repairs when (and if) necessary during the entire length of the contract. This includes full coverage for parts, labor and travel; Reagent replacement upon HW failures; Remote Technical Support; 3 business day average on site response; HW/SW updates; On site applications support. Deliverables: One preventative maintenance per year, which includes labor, travel and replacement of all parts included in the PM Kit; Component Inspections; Subsystem Maintenance; System Requalification; and a PM Certification document. Other Pertinent Information or Special Considerations: All service technicians and support technicians must be FACTORY TRAINED AND CERTIFIED SERVICE ENGINEERS. They must be trained and certified by Illumina Inc. personnel and should be continuously trained on the latest instrument technology, product improvements, software, and accessories. Inspection and Acceptance Criteria: The Contracting Officer Representative (COR) or his/her designee shall be responsible for certifying that the work done under the contract is performed to time and standard. They are also responsible to assure the inspection and acceptance of products provided incidental to services. A demo period or trial is not inspection of a product. Risk Control: Access to all areas of the research building requires that the contract be escorted to the work site and contractor must comply with all signage posted with regards to safety procedures. Type of Contract: Firm Fixed Price (FFP) Place of Performance: James J. Peters VAMC Research Building 130 West Kingsbridge Road Bronx NY 10468 Period or Performance: April 1, 2017 through March 31, 2018 April 1, 2018 through March 31, 2019 April 1, 2019 through March 31, 2020 April 1, 2020 through March 31, 2021 April 1, 2021 through March 31, 2022 Delivery Schedule: Initial visit to be scheduled within 14 calendar days of issuance of contract and then scheduled as needed Security Requirements: Vendor personnel must complete VA Privacy and Information Security Awareness training. This is a mandatory training for all VA and Non-VA employees working at this facility. This training must be completed annually. Completion certification must be provided for each employee that will be onsite. Vendor is required to coordinate all work with POC/COR assigned. Smoking is prohibited inside/outside any building other than the designated smoking areas. The contractor shall comply with safety standards, furnished by the VA Safety Office and for instructing employees in safety measures weekly as specified in Occupational and Environmental Safety, Fire Protection and Health Standards. Research department requires that all visitors and contractors be escorted and supervised while on site. Responses to this notice must be submitted in writing via email and must be received not later than Tuesday, March 7, 2017, 1:00pm EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of applicable certification. DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business).
