Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89 issued and effective July 14, 2016. This solicitation VA259-17-Q0260 is issued as a request for quote (RFQ). The North American Industry Classification System Code (NAICS) for this procurement is 337127. This RFQ is being issued as a Service Disabled Veteran Owned Small Business set-aside under NAICS Code 337127, SBA Small Business Size Standard is 500 employees. Any Award Made as a Result of this Solicitation will be made on an All or Nothing Basis. All Items being Solicited (Products and/or Receivables as appropriate) must be of New Manufacture; No Remanufactured or "Grey Market" items allowed. FAR 52.212-4 applies to all Solicitations and Awards. Department of Veterans Affairs, VISN 19, VA Medical Center, 921 NE 13th Street, Oklahoma City, Oklahoma 73104, is seeking a quote for the items listed below, in accordance with the furniture requirements identified below. Solicitation and evaluation of offers are to be in accordance with (IAW) FAR 11.105 (a)(1). The STATEMENT OF WORK (SOW) for the requirements is as follows: 8 North Day Room and Employee Breakroom Furniture 1. Purpose Purchase, Delivery and Installation of furniture for OKC VA 8th floor North Patient Day Rooms and Employee Breakroom (new furniture). Remove existing furniture: 9-tables and 45- chairs to VA Medical Center Oklahoma City (OKC) excess, located west of, adjacent to hospital. 2. Background Furniture is required for the 8th floor North In-Patient Behavioral Health Unit of the OKC VA Medical Center. This furniture is for the behavioral healthcare environment. OKC The furniture is required to create a safe, secure and suicide prevention environment for the patients. 3. Weight, Base Moss, Color Pine Cone The following applies to the items listed above: Warranty: 10 year Limited Replacement Warranty 4. Placement Requirements The Point of Contact (POC) shall provide 8th floor North Patient Day Rooms and Employee Breakroom furniture plan to contractor upon delivery of furniture. 5. Security Requirements All work will be coordinated with the Oklahoma City VA Health Care System s POC. Installation schedule 8am 4 pm Monday through Friday. 6. Delivery Schedule - VA Health Care System can take immediate delivery of all items listed on order. Contractor shall schedule delivery within 1 week after the contract award, approved by the POC. Delivery can be made to the loading dock Monday through Friday 7:00am to 4:00pm (except for Federal holidays) at: VA Health Care System, 921 NE 13th Street, Oklahoma City, OK 73104 7. Federal Holidays: Federal holidays are any calendar days designated by statute or executive order as federal holidays. Federal holidays consist of New Year s Day, Martin Luther King Jr. s Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas. 8. Protection From Damage: Contractor shall provide all labor, materials, and equipment necessary for protection of VA Healthcare System (HCS) personnel, furnishings, equipment and buildings from damage, and shall replace or repair any damage due to work performed under this contract equal to its original construction and finish. 9. Response Requests: Contractor shall only respond to requests for service from VA HCS Contracting Officer (CO) or POC. If service is requested from any other source the Contractor is to advise the caller of this requirement and refuse to respond. Any billing resulting from unauthorized service will not be paid by the government. 10. Check In/Out Procedures: Prior to commencement of, and immediately after each service visit, the Contractor or his authorized repair technician must check in and out with the VA HCS POC (during normal working hours). Due to heightened security requirements, the Contractor or his authorized service representative will be required to be escorted by VA personnel designated by the POC.
