Combined Synopsis/Solicitation Solicitation VA248-17-Q-0456 (i) This is a combined synopsis/solicitation for commercial items is prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation with quotes being requested from capable responsible contractors and a written solicitation will not be issued. The Department of Veterans Affairs, James A. Haley Veteran s Hospital (JAHVH), 13000 Bruce B Downs Blvd., Tampa, Florida 33612 has a need to procure Maintenance and Repair Services of Prismaflex Continuous Renal Replacement Therapy (CRRT) Machines. (ii) The combined synopsis solicitation number is issued as Request for Quotation (RFQ) # VA248-17-Q-0456. (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) Number/Effective Date 2005-95/01-20-2017 (iv) The combined synopsis solicitation is a Veteran Owned Small Business (VOSB) Set-Aside. The NAICS is 811219 Medical and Surgical Equipment Repair and Maintenance Services. Business size standard is $20.5 Million. (v) This combined synopsis solicitation is for Prismaflex Continuous Renal Replacement Therapy (CRRT) Machine Preventative Maintenance & Repair Services. Expected period of performance is for a base year followed by four consecutive one year option periods. Vendors shall use the Pricing Cost Schedule, which includes all contract line item numbers (CLINs), quantities and units of issue, to fill in all unit prices and total amount blocks. The Government reserves the right to make no award if no quote meets the requirements of the solicitation. (vi) A description of the services to be acquired, and/or minimum Government need(s): The Contractor shall provide full maintenance services (preventative and corrective) for Prismaflex CRRT Machines as found in Section 2 Equipment List of the Statement of Work (SOW). This requirement includes the machine software, all maintenance services (including corrective and preventive maintenance), supervision, personnel, equipment, transportation, tools, materials, parts and services necessary to maintain the Prismaflex Machines in proper operating condition and assure optimum performance and reliability as well as the overall safety of the patients. (vii) The place of performance and acceptance is JAHVH, 13000 Bruce B. Downs Blvd., Tampa, FL 33612. (viii) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items, applies to this acquisition and is incorporated by reference. An addendum to this provision is included. Quoters are cautioned to follow these instructions carefully. (ix) The provision at FAR 52.212-2, Evaluation Commercial Items, does not apply to this acquisition. See Section D.1, Basis for Award/Comparative Evaluation for the evaluation procedures to be used and the basis for award. (x) 52.212-3 Offeror Representations and Certifications Commercial items (DEC 2016) applies to this acquisition. Quoters shall include a completed copy of the provision at 52.212-3 Offeror Representation and Certification Commercial Items (Oct 2016) with their quote submission. LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011) This solicitation includes FAR 52.219-6 Notice of Total Small Business Set-Aside. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.
