Project: Survey Questionnaire for Rehabilitation Programs Background: The Department of Veterans Affairs (VA) is committed to providing specialized treatment and rehabilitation care to Veterans with disabilities. These populations include, but are not limited to, Veterans with spinal cord injury and disorder (SCI&D), blindness or severe visual impairment, traumatic brain injury, amputation, other orthopedic conditions, serious mental illnesses, and homeless Veterans. As part of our obligation to monitor and assure high quality care to these groups, a valid survey of patient experiences during rehabilitation was launched in 2011. The rehabilitation survey compliments surveys administered to general VA inpatient and outpatient services that are currently monitored with the Survey of Health Experiences of Patients (SHEP). Given that the underlying instrument that forms the basis of SHEP, the Consumer Assessment of Health Providers and Systems (CAHPS), was not developed to address the specific needs of patients undergoing rehabilitation and use of a separate and distinct rehabilitation survey was necessary. VA rehabilitation programs currently are using the CARF developed uSPEQ survey to obtain valid information from their patients on such key areas as: 1) service responsiveness; 2) informed choice; 3) respect of individual needs; 4) functional outcomes and community participation; 5) overall perceived value and 6) respondent demographics. Without continuing such information collection, and the ability to benchmark with leading rehabilitation facilities within and outside of VA, we are limited in our ability to do continuous quality improvement, provide informed programmatic development and evaluation, rapidly identify rehabilitation program strengths and weaknesses, or conform to accreditation agency standards. Purpose: The purpose of this requirement is to: Provide the means to administer a tested, statistically valid and reliable rehabilitation customer experience survey within VA medical and psychosocial rehabilitation programs. This includes access to paper survey forms and web-based surveys as deemed appropriate for the specific patient population served at a program site. Content areas of current surveys include Service Responsiveness, Informed Choice, Respect, Participation, Overall Value, and Demographics. VA programs currently surveyed include Blind Rehabilitation, Homeless Programs, Medical Rehabilitation and Spinal Cord Injury. Others may be added if the need and resources are present. Provide program-specific results, along with internal VA and external non-VA benchmarking comparisons, for medical and psychosocial rehabilitation programs administering the rehabilitation survey. Information Requested: The contractor should illustrate its ability by providing supportive documentation that describes past projects and current capabilities that are of similar size, scope, and complexity. Past project information shall include (if applicable) contract/order number, type of contract (i.e., FPP), customer s point of contact information (name/phone/email, position), procuring contracting officer s email address, and contract value. Specific Questions to be Addressed (all questions listed below must answered completely): Describe your capacity/capability to provide/performance Survey Questionnaire for Rehabilitation Programs requirements for VA rehabilitation patients to support healthcare programs in similar size, scope and complexity as described in the attached draft PWS? Identify and describe the associated risk with the complexity of this requirement. Describe your organizations experience with surveys in a rehabilitation setting, specifically veteran s rehabilitation. The Government has determined the North American Industry Classification System (NAICS) code 541611-- Administrative Management and General Management Consulting Services is the most appropriate for this require. If the contractor believes another code would be more appropriate, please list the code and justification. Identify the number of labor hours, labor categories, and any other relevant pricing components/industry standards to consider, that support tasks as described in the attached draft PWS. Include a rationale/justification for a suitable contract type, if other than Firm Fixed Price. Please specify your organizations social economic category. Please provide any suggestions or feedback for the attached draft PWS.
