- Conformed Solicitation Amendment 5 Deliverables Schedule Question and Answer Spreadsheet Performance Risk Questionaire Sections L and M Quality Assurance Surveillance Plan The purpose of this modification is to: 1. Extend the Past Performance Questionnaire and proposal submission date from 10 March 2017 to 17 March 2017. 2. Provide answers to questions received by 25 January 2017. 3. Provide a revised Performance Work Statement. 4. Remove Section L and M from solicitation and add the revised Sections L and M as an attachment. 5. Delete Tech Exhibit 1 - Perf Reqmts Summary and renumber Tech Exhibit 2 - Deliverables to Attachment 5. 6. Add a Quality Assurance Surveillance Plan as an attachment. All other terms and conditions remain unchanged. DO NOT PROPOSE ON CLIN 0002, 1002, 2002, 3002, 4002 WEB HOSTING. CLIN HAS BEEN DELETED.
