- Combined Synopsis This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6, Streamlined Procedures for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Quotes are being requested and a written solicitation will not be issued. The solicitation number is W81XWH17T0127. The solicitation is issued as a request for quotation (RFQ). Provisions and clauses in effect through Federal Acquisition Circular 2005-95 are incorporated. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses https://acquisition.gov/far/index.html and http://www.acq.osd.mil/dpap/sitemap.html. The NAICS code is 325199, with a small business size of 1,250 employees. This is a Full and Open Competition requirement; all qualified vendors are encouraged to submit a quote. USAMRAA on behalf of the Armed Forces Medical Examiner System (AFMES) requests responses from qualified sources capable of providing Brand Name or Equal to Synthetic Cannabinoids in accordance with the minimum essential characteristics (MECs), and Statement of Need (SON). The resultant Blanket Purchase Agreement (BPA) will be on a DD 1155 which will not include any Contract Line Item Numbers (CLINS). Therefore, there are no CLINS included on this soliciations. The Government intends to award one Blanket Purchase Agreement (BPA) contract, but may award multiple. Requirements will be procured via the award of call orders issued against the basic agreement(s). The minimum for each awardee is $500. 00. The maximum value of all call orders issued as result of this solicitation is $1,200,000.00 Period of Performance: Base Year - est: May 2, 2017 - May 1, 2018 Option Year 1 - est: May 2, 2018 - May 1, 2019 Option Year 2 - est. May 2, 2019 - May 1, 2020 Option Year 3 - est: May 2, 2020 - May 1, 2021 Option Year 4 - est: May 2, 2021 - May 1, 2022 Customer Location: 115 Purple Heart Drive Dover AFB, DE 19902 INSPECTION: Unless otherwise specified in the order, the offeror is responsible for the performance of all inspection requirements and quality control. Other Administrative Instructions: System for Award Management (SAM). Offerors must be registered in the SAM database to be considered for award. Registration is free and can be completed on-line at https://www.sam.gov/portal/public/SAM/; provide DUNS number; Cage Code and TIN. Offerors are reminded to include a completed copy of 52.212-3 ALT I with RFQ response. All clauses shall be incorporated by reference in the order.
