SOURCES SOUGHT Biological Hoods Inspection The Department of Veterans Affairs, Network Contracting Office 10, 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200, Ann Arbor, MI 48105, intends to solicit offers for Biological Hoods Inspection with all labor, materials, equipment, supervision, and all necessary resources to meet the requirements below. The requirements shall be performed at the John D. Dingell VA Medical Center, 4646 John R Street, Detroit, MI 48201. Contractors that can provide this requirement and meets the requirements below are requested to send their 1. Company Name, 2. DUNS number, 3. Business size under NAICS 811219 and 4. If the item(s) are available on a FSS contract or not and the FSS contract number if it is available on contract, 5. Notification if they are a VETBIZ registered VOSB/SDVOSB, SMALL BUSINESS, WOMEN OWNED SMALL BUSINESS, COMPETETIVE 8 (a), or not. This is not a solicitation. Statement of Work Hood Certification Contract FY17 GENERAL INFORMATION Title of Project: Hood Testing and Certification Contract Detroit VA Medical Center is looking for one base year contract and four option years contract bid. Scope of Work (Semi-Annual): The contractor shall provide all resources necessary to accomplish the deliverables describe in this statement of work (SOW), except as may otherwise be specified. The contractor shall provide all labor, material, instrumentation, supervision, and all other resources required to perform semi-annual testing and certifications on: 15 Biological Hoods 2 Laminar Flow Hoods 16 Chemical Fume Hoods 1 Pharmacy IV Room These rooms are located in PALMS, Pharmacy, Dental Lab, Sickle Cell Lab, and Nuclear Medicine. All testing and certification will be done based on any VA standards, Joint Commission requirements, Accreditation of Hospital Organization, or National Codes. The Pharmacy IV clean room environment shall meet certification requirement set forth in the USP797 standard. Scope of Work (Annual): The contractor shall provide all resources necessary to accomplish the deliverables describe in this statement of work (SOW), except as may otherwise be specified. The contractor shall provide all labor, material, instrumentation, supervision, and all other resources required to perform annual testing and certifications on: 26 Biological Hoods 2 Laminar Flow Hoods 39 Chemical Fume Hoods These rooms are located in the lower level Animal Research, and 4th floor Research area. Background Semi-Annual: Semi-Annual hoods and the IV Clean room are to be tested and certified for proper airflows on an annual and semi-annual basis. The contractor shall be required to test and certify all hoods and the IV Clean room within the John D. Dingell VAMC. The contractor shall perform these tests in November and May for the semi-annual testing and certifications. The contractor will schedule the work to make sure we do not have more than a 6-month overlap of inspections in the contracts fiscal year. Background Annual: Annual hoods are to be tested and certified for proper airflows on an annual basis. The contractor shall be required to test and certify all hoods within the John D. Dingell VAMC. The contractor shall perform these tests in November testing and certifications. Performance Period: The contractor shall complete the work required under this SOW within the month of November and again in the month of May of the fiscal year in which the contract has been awarded. The work shall take place during normal operating hours from 6:00am-4:30pm Monday thru Friday. Some critical areas may be requiring testing to be scheduled a day ahead of time in order to gain access. This will be scheduled thru the COR. The contractor shall complete all testing within five business days from the start date of the initial work. Type of Contract: This will be a firm-Fixed Price contract. SPECIFIC MANDATORY TASK AND ASSOCIATED DELIVERABLES The contractor shall provide the specific deliverables below within the performance period stated in Section A-4 of this SOW. The contractor shall start the first of the semi-annual testing/certification of all the hoods and IV clean room in the month of November of the contract year in which it is awarded and be completed within 5 business days from when the work was started. The contractor shall start the second of the semi-annual testing/certification of all the hoods and IV clean room in the month of May of the contract year in which it is awarded and be completed within 5 business days from when the work was started. The contractor will give the Contracting Officer Representative (COR) at least one week noticed before starting any testing. The contractor shall communicate daily during the inspections with the COR to let him know if there are any issues with the hoods tested that day. The contractor shall notify the COR of any hoods that do not meet certification criteria and supply the COR with a quote for repairs needed to make the hood compliant prior to leaving the facility. Testing of the Pharmacy IV clean room shall be done on a semi-annual schedule to coincide with the hood testing. The following tests are required: Air Flow Supply and Return CFM: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: To determine the volume of air (in CFM) delivered through each HEPA filter and to calculate the average airflow within the cleanroom area designation. HEPA Filter Leak Test: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: To verify that all HEPA filters filter housing's and mounting frames located in the clean room are free from leakage when operating at manufacturers recommended airflow velocities. Airborne Particle Count: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: To measure the airborne particle levels in the Cleanroom in order to classify the room per ISO/TC209 14644-1 Airborne Particulate Cleanliness Classes. Temperature and Humidity Tests: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: The purpose of these tests is to demonstrate the capability of the system to control temperature uniformity and humidity within buyer-specified limits. Sound Levels: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: The purpose of this test is to establish the airborne sound pressure levels produced by the basic Cleanroom mechanical and electrical systems as experienced within the room and in adjacent external occupied areas, and to verify that performance meets the Buyer's specifications. Light Level: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: The purpose of this test is to verify that the specified lighting levels and uniformity of lighting within the Cleanroom has been met. Room Pressurized Test: Ante Room, I.V. Room, Air Lock, and Chemo Room Purpose: To verify the capability of the Cleanroom system to maintain the specified pressure differential in the Cleanroom. Locations of all areas and test locations will be required for each individual test. CONFORMANCE STANDARDS All testing of hoods are to meet any VHA standards and Joint Commission for Accreditation of Hospital Organization or National Codes. The Pharmacy IV clean room environment shall meet certification requirement set forth in the USP797 standard. D. HOURS OF WORK Hood testing will be performed during normal working hours Monday thru Friday starting at 6:00am-4:30pm. The Pharmacy IV clean room testing will be done during normal working hours 6:00am-4:30pm Monday thru Friday. Any time changes must be cleared thru the acting COR of the contract. All Government Federal Holidays are observed by the VA hospital. Any work during these days must be cleared thru the COR of the current contract. E. LOCATIONS OF SERVICES John D. Dingell VA Medical Center 4646 John R. Street Detroit, MI. 48021 F. GOVERNMENT RESPONSIBILITIES The Government will provide access to the rooms where hoods are located so the contractor can perform testing/certifications. The Government will supply room maps for all areas of work. The COR will provide an escort if needed to find rooms and locations for the contractor. G. SAFETY The contractor shall provide any and all personal protective equipment (PPE) their employees need such as safety glasses, hearing protection, safety shoes, gloves etc.. H. REPORTS The contractor shall be responsible for submitting detailed reports within 14 days of the completion of each semi-annual inspection for each type of hood. The report is to include the type of hood, ID number, EE number, room number, air flow data and hood pass/fail.
