Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number VA256-17-Q-0391 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. Offers are due no later than Thursday, March 16, 2017 by 2:00 p.m., Central Local Time (CST), New Orleans, LA. This requirement is for Brand Name only stated in the solicitation. This solicitation is set-aside 100% for Service-Disabled Veteran-Owned Small Business concerns, under NAICS code 334510 with a small business standard of 1,250 employees or less. PURPOSE 1.1 The purpose is to provide Sonomed VuMAX Probes to the Southeast Louisiana Veterans Health Care System (SLVHCS) Surgery Services (Outpatient Ophthalmology Clinic) located at 2400 Canal St, New Orleans, LA 70119. 2. SCOPE 2.1 The Contractor shall provide all listed equipment. All products must meet all salient characteristics defined in this section. 2.2 SALIENT CHARACTERISTICS 2.2.1 20 MHz B-Scan Probe BRAND NAME: Sonomed Brand Name Only; No Equivalent or Substitute Products Specifications/Salient Characteristics: 20 MHz probe B-Scan probe that is capable of performing posterior B-Scan Capable of transmitting data from 256 ray scan with 2048 sample points for each ray per transducer sweep Capable of capturing 50-frame video clips at up to 20 frames per second Must be compatible with existing Sonomed VuMAX HD 2.2.2 35 MHz UBM Transducer BRAND NAME: Sonomed Brand Name Only; No Equivalent or Substitute Products Specifications/Salient Characteristics: Motor driven interchangeable Ultrasonic Bio-Microscope transducer Transducer frequency of 35 MHz Must be capable of variable field sector scanner Must be capable of sector angles of 38 degree or 20 degree fields Variable scanning speed of 12.5 frames per second Variable observable range with the following capabilities : 18.5 mm W x 14mm D in 38 ° field 12.0 mm W x 14mm D in 20 ° field Acoustic axial resolution of 0.068mm Must be responsive to user selectable contrast and brightness Capable of angle to angle, sulcus to sulcus, corneal thickness, and scleral thickness distance measurements Capable of single measurement of cornea thickness, anterior chamber depth, and lens thickness anterior segment biometry Must be compatible with existing Sonomed VuMAX HD 2.2.3 50 MHz UBM Transducer BRAND NAME: Sonomed Brand Name Only; No Equivalent or Substitute Products Specifications/Salient Characteristics: Motor driven interchangeable Ultrasonic Bio-Microscope transducer Transducer frequency of 50 MHz Must be capable of variable field sector scanner Must be capable of sector angles of 38 degree or 20 degree fields Variable scanning speed of 12.5 frames per second Variable observable range with the following capabilities : 18.5 mm W x 14mm D in 38 ° field 12.0 mm W x 14mm D in 20 ° field Acoustic axial resolution of 0.068mm Must be responsive to user selectable contrast and brightness Capable of angle to angle, sulcus to sulcus, corneal thickness, and scleral thickness distance measurements Capable of single measurement of cornea thickness, anterior chamber depth, and lens thickness anterior segment biometry Must be compatible with existing Sonomed VuMAX HD 2.2.4 Soft-Touch A-Probe BRAND NAME: Sonomed Brand Name Only; No Equivalent or Substitute Products Specifications/Salient Characteristics: Direct contact A-Scan Capable of being utilized for manual or automatic capture Capable of transmitting auto calculation of axial length, anterior chamber depth, lens thickness, and vitreous Capable of transmitting data in Standard IOL formulas such as Binkhorst, Regression-II, Theoretic/T, Holladay, Hoffer-Q, and Haigis Capable of transmitting post refractive IOL data including Latkany Myopic Regression, Latkany Hyperopic, and Aramberri Double-K Must be compatible with existing Sonomed VuMAX HD 2.2.5 Water-Path Probe BRAND NAME: Sonomed Brand Name Only; No Equivalent or Substitute Products Specifications/Salient Characteristics: Capable of immersion A-Scan Capable of being utilized for manual or automatic capture Capable of transmitting auto calculation of axial length, anterior chamber depth, lens thickness, and vitreous Capable of transmitting data in Standard IOL formulas such as Binkhorst, Regression-II, Theoretic/T, Holladay, Hoffer-Q, and Haigis Capable of transmitting post refractive IOL data including Latkany Myopic Regression, Latkany Hyperopic, and Aramberri Double-K Must be compatible with existing Sonomed VuMAX HD 3. DELIVERY 3.1 Contractor shall deliver all equipment to the Southeast Louisiana Veterans Health Care System (SLVHCS) Surgery Services 2400 Canal St, New Orleans, LA 70119 on June 1, 2017. 3.2 Deliver materials to job in manufacturer's original sealed containers with brand name marked thereon. 3.3 Package to prevent damage or deterioration during shipment, handling, and storage. 3.4 Maintain protective covering in place and in good repair until delivery is necessary. 3.5 Any government requested delayed delivery up to 90 days after initial award delivery date, shall be at no additional cost to the Government. 4. INSPECTION AND ACCEPTANCE: 4.1 The Contractor shall conduct a joint inspection with the COR upon delivery of tools. 5. DELIVERABLES 5.1 Operation and Maintenance Manuals 5.1.1 Binders - Quantity (2) each for items 2.2.1 5.1.2 Digital Copies- Quantity (1) each for items 2.2.1 5.2 Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to COR upon completion of installation. 6. SECURITY REQUIREMENTS In accordance with Handbook 6500.6 Contract Security (March 12, 2010) include this contract security language into the Statement of Work (SOW) immediately following the security clause section: "A&A requirements do not apply--Security Accreditation Package is not required". 6.1 GENERAL Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. 7. WARRANTY: 7.1 The contractor shall provide all manufacturers warranty with products upon delivery. The following FAR provisions and clauses apply to this solicitation and are incorporated by reference. The solicitation document, incorporated provision and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, Effective January 13, 2017. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions.
