- This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures for commercial items found at FAR 13, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on FEDBIZOPPS. Naval Medical Center San Diego (NMCSD) has a requirement for a one (1) year Full Service Maintenance Agreement contract for government owned equipment, Spectranetics CVX-300 Excimer Laser System. The service agreement as defined in the Statement of Work (attached) to include all parts, labor, transportation, and unlimited maintenance/service calls (corrective and preventative) from 01 April 2017 to 31 March 2018 with additional two (2) option years. The Request for Quotation (RFQ) number is N0025917Q0095. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-95 and DFARS Publication Notice 20161222. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The NAICS code is 811219 and the Small Business Standard is $20,500,000. This requirement is for Full and Open Competition. The Naval Medical Center San Diego (NMCSD) requests responses from qualified sources capable of providing the restock of the following items that must meet the salient characteristics described above: CLIN 0001 CVX-300 Excimer Laser system $_______per quarter $_________per year S/N 80800 Base Year-Maintenance Service Agreement POP: 01 Apr 2017-31 Mar 2018 Service Plan Agreement for the listed equipment will be provided in accordance with the Statement of Work. CLIN 1001 CVX-300 Excimer Laser system $________per quarter$_________per year S/N 80800 Option Year 1-Maintenance Service Agreement POP: 01 Apr 2018-31 Mar 2019 Service Plan Agreement for the listed equipment will be provided in accordance with the Statement of Work. CLIN 1002 CVX-300 Excimer Laser system $_______per quarter $_________per year S/N 80800 Base Year-Maintenance Service Agreement POP: 01 Apr 2019-31 Mar 2020 Service Plan Agreement for the listed equipment will be provided in accordance with the Statement of Work. Location is 34800 Bob Wilson Drive San Diego, CA 92134.
