Description

The Department of Veterans Affairs, VISN 22 - Network Contracting Activity Office is seeking Contractors interested in providing Mortuary and Transportation services and supplies of unclaimed remains for the VA San Diego Healthcare System, 3350 La Jolla Village Drive, San Diego, CA 92357. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 812210 Funeral Homes and funeral Services where the Small Business Size Standard is $7.5 Million. This requirement is 100% set-aside for Service-Disabled Veteran-owned Small Business concerns. In accordance with VAAR 852.219-11, any SDVOSB submitting a quote for this action must be verified for ownership and control and who is so listed in the vendor information pages database (http://www.vetbiz.gov) upon submission of quote. The Contractor shall be required to furnish all necessary and appropriate labor, supervision, materials, equipment, transportation, and supplies to provide 1. Preparation of the body: Embalming and refrigeration. 2. Clothing: The deceased Veteran s body will be dressed in suitable and appropriate attire. Veterans shall be dressed for burial. 3. Casket: The casket, at a minimum, must be constructed from thick, strong particleboard and must be of sufficient strength to support the weight of an adult human body. Cardboard or press paper or similar materials are not acceptable. 4. Permits: Mortuary Company will secure all necessary permits to provide service. Reimbursement for such permits will be included in transportation charges. 5. United States Flag: The Mortuary will ensure that a U.S. Flag, provided by the Funeral Director at the VA, accompanies the casket to the place of burial. 6. Transportation: The body will be transported by hearse or van from the VA facility to the funeral home and to the place of burial. It is the Contractor s responsibility to review this FBO website for amendments, updates, or changes to current information. Submit to the Contract Specialists attention, sylvia.correa@va.gov any requests for information or clarification of the solicitation documents, no later than March 15, 2017. Complete proposals shall be submitted electronically to the attention of sylvia.correa@va.gov. Please fill the attached Solicitation and submit on or before March 17, 2017, not later than 2:00 p.m. Pacific Standard Time (PST). The email subject line shall include the solicitation number and title when submitting your proposal.
