It is awarded for a Service Disabled Veteran Owned Small Business (SDVOSB) Sole Source as prescribed in FAR 6.302-5, Authorized or Required by Statute. Award Number: VA262-17-P-2981 Award Amount: $157,258.00 Award Date: 03-01-2017 Contractor s DUNS Number: 079613350 Awardee Name and Address: Aviate Enterprises, Inc. 5844 Price Ave McClellan, CA 95652 Prior to this notice, market research was conducted and concluded that Aviate Enterprises, Inc. is a Certified Service Disabled Veteran Owned Small Business (SDVOSB) in the www.vetbiz.gov database. In addition, Aviate Enterprises, Inc. has been a model SDVOSB and has provided this identical service to other Veteran Affairs Medical Center (VAMC) and has established that in addition to providing the supplies being requested, it has the capabilities to provide installation, service & training. This sole source action is being acquired in accordance with the statue identified under Public law 109-461 Title 38 Veteran s Benefits U.S. Code 8127, which allows sole source contracts above the simplified acquisition threshold to be awarded to a small business owned and controlled by veterans and small business concerns owned and controlled by veterans with service-connected disabilities by using other than competitive procedures. This requirement is to provide Mobile CT Scanner for 5-month period and below are the Salient Characteristics/SOW required for this request: Contractor shall provide a 64-slice CT Scanner mounted in a mobile specialty vehicle in compliance with all applicable safety codes and regulations. Contractor shall provide a site planning guide for installation and operation of mobile scanner. Electrical Service Unit shall operate on 480 Volts, 3 Phase, 150 Amps using a five wire system. Mobile CT shall have a 40 foot cable with a male connector. If a generator is provided on the unit it must be permitted with the Air Pollution Control District (CARB) in accordance with all local and state requirements. Wheelchair Lift Provide a wheelchair lift for patient access to the mobile CT with all required safety tie downs and guards. Support The unit shall be self supporting and have a rear stabilizer stand and adjustable jack legs in front to accommodate leveling on a site that is flat but on a slight sloped grade. Telephone and Data Interface Mobile CT Unit will have telephone and data hook-ups to interface with facility s telephone and IT/PACS Systems. Medical Device Operating Systems For all medical devices (i.e. CT Scanner), the operating systems must be loaded with Windows 7 or newer and or Windows Server 2008 or newer per VA Directive 6550, Pre-Procurement Assessment for Medical devise/system. Mobile CT Unit will have a CT contrast power injector. Hard Drive Replacement Hard drives to be removed by staff BioMed personnel and replaced by contracted vendor at an agreed to cost. Contractor will provide an orientation to the operation of the components of the mobile specialty vehicle including safety features, operation of the wheelchair lift, location of emergency off switch, and any other features required for patient and employee safety. The contractor will deliver the equipment to VA Loma Linda Healthcare System (VALLHCS) on March 13, 2017. This procurement is being conducted using commercial item procedures pursuant to FAR Part 12 and FAR part 13.
