Description

Statement of Work Title Service on site for evaluation of VA owned OCT (Ocular Coherence Tomography Unit ) S/N SPEC-CAM-04827-S3600, VA ID# 35-101718 Scope Vendor shall come on site to install loaner OCT and ship VA owned OCT to the Heidelberg repair facility. Background OCT was purchased in June of 2012. OCT has been maintained by in house Biomed staff since purchase. OCT is used routinely in the Eye Clinic on 95% of patients examined. Staff reported the OCT was not working and was making a strange noise. Biomed personnel confirmed the OCT is broken and requires OEM repair. Performance period Vendor shall to ship and install loaner within 5 business days of receiving purchase order from the VA.
