Page 1 of 2 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures, specifically FAR Subpart 13.5 Simplified Procedures for Certain Commercial Items; and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation is attached to this announcement. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-95, dated January 19, 2017. Solicitation # VA262-17-Q-0091 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award from this solicitation. The North American Industry Classification System (NAICS) Code is 561320 and the Small Business Administration s (SBA) size standard is $27.5 million for this procurement. This procurement will be issued as a total set aside for Service Disabled Veteran Owned Small Businesses (SDVOSB). As such, offerors must be verified as a SDVOSB under the applicable NAICS on the VA Center for Verification and Evaluation s website located at https://www.vip.vetbiz.gov. Description of Services: The Government is seeking a contract for one (1) Full Time Equivalent (FTE) Certified Medical Dosimetrist (CMD) to provide onsite radiation therapy services at the VA Greater Los Angeles Healthcare System (VAGLAHS) Radiation Therapy Service. Onsite radiation therapy services shall meet or exceed any current or future requirements approvals or certifications as may be levied by applicable legal, regulatory, accrediting body, governing body, and/or local/state/federal requirements as required by law (e.g. American College of Radiology Guidelines for Radiation Oncology, Veterans Affairs standard operating procedures, etc.). The CMD must be board certified and in good standings with the Medical Dosimetrist Certification Board (MDCB), www.mdcb.org. The CMD shall meet all the qualifications and be able to perform in accordance with the solicitation requirements and performance work statement (PWS). Please see attached SF 1449 Solicitation for Performance Work Statement (PWS), applicable clauses / provisions, and other pertinent information relative to this solicitation. Place of Performance: VA Greater Los Angeles Medical Center (VAGLAHS), Radiation Therapy Service, 11301 Wilshire Blvd, Los Angeles, CA 90073. Period of Performance: Base year and up to two (2) one (1) year option periods. Provisions 52.212-1, Instructions to Offerors Commercial, 52.212-2, Evaluation Commercial Items, 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items are applicable to this acquisition. PLEASE SEE SECTION E OF THE SF 1449 FOR SPECIFIC INSTRUCTIONS. Furthermore, Provision 52.212-2 is utilizing a best value evaluation process, as such, the Government will award a contract resulting from this solicitation to the Offeror whose offer conforms to the solicitation and is determined to be the best value to the Government, price and other non-price factors considered.
