- FA4486-18-R-0001, Grounds Maintenance Solicitation Attachment 4 Site Visit Reservation Form Attachment 3 Past Performance Questionnaire Attachment 2 Proposal Schedule 9 March 2017 Attachment 1 Performance Work Statement 8 March 2017 Lajes Field, Terceira, Azores, Portugal has a requirement for Grounds Maintenance. The Government intends to award a single award firm fixed price contract using FAR Part 12, 13, and 15. The contract term will be a base period of eleven (11) months plus four (4) one (1) year options; beginning on 1 November 2017 to 30 September 2018, which will be included in the evaluation for award. This is a solicitation for the following: Non-personal service, to include all personnel, equipment, tools, supervision, and other items and services necessary to ensure that grounds maintenance is performed at Lajes Field, Azores, Portugal and off Base support sites in a manner that will promote the growth of healthy grass, trees, shrubs, and plants and present a clean, neat, and professional appearance through the growing season, which for Lajes Field consists of 52 weeks a year. The North American Industry Classification System (NAICS) Code for this procurement is 561730. A pre-proposal site visit will be conducted at the 65th Contracting Flight, Building T-615, 2nd Floor on 22 March 2017 at 09:00AM AZOST. Interested vendors must submit their site visit attendees information (Attachment 4) no later than 20 March 2017 by 4:00PM AZOST. Offers may be submitted by hand receipt, mail or email in response to this solicitation. The content and page size of electronic copies must be identical to the hard copies. Facsimile submissions will not be accepted. The closing date for receipt of offers will be 10 April 2017, 4:00PM AZOST to the following email addresses: 65CONF.LGC.ALL@us.af.mil and andrea.silva.1.pt@us.af.mil.
