SOLICITATION W81K00-17-T-0143 Attached is a solicitation for the maintenance and service of Wizard2-1-Detector gamma counter equipment with the performance to occur at Evans Army Community Hospital, Fort Carson, Colorado Springs, CO, 80913. In order to proceed with the proposed contract I need you OR the appropriate person to: 1. Fill out pricing for the all of the line items included in the solicitation. - Line item 0001: Here, you would put the total cost of the requested item. - Line item 0002: Here, if required, you would put the total cost of the requested item. - Line item 1001: Here you would put the total cost of the requested item. - Line item 1002: Here, if required, you would put the total cost of the requested item. -Line item 2001: Here you would put the total cost of the requested item. - Line item 2002: Here, if required, you would put the total cost of the requested item. -Line item 3001: Here you would put the total cost of the requested item. - Line item 3002: Here, if required, you would put the total cost of the requested item. -Line item 4001: Here you would put the total cost of the requested item. - Line item 4002: Here, if required, you would put the total cost of the requested item. 2. Please fill out block 17a with the company CAGE Code. 3. Sign and date in blocks 30 a, b, and c. Please return the completed solicitation NLT 17 March 2017 by 10:00 a.m. (CST).
