Description
VA263-17-Q-0420_1 Questions & Answers The purpose of this amendment is to answer contractor questions. All other terms and conditions will remain unchanged. Q1: Who is the Radiation Safety Officer and is he responsible for all three sites? Will he be available during testing ? A1: Pam Cox is a contracted Radiation Safety officer for all three sites and is normally not on site. The radiation safety officer is typically not present as most of the surveys are done off normal duty hours for ease of access to equipment. Q2: Are there dates available for when the equipment was last tested? A2: All equipment was last tested in Sep 2016 Q3: Will testing of any new or repaired equipment, shielding of rooms etc be included? A3: No, these situations will be done on a separate PO as they come up. Q4: Who will notify the COR or CO for unusual or non-contract procedures that are requested? A4: Will need some examples of unusual or non-contract procedures. The person doing the radiation output measurements will deal with an on-site biomed tech who will contact the COR in Omaha Biomed department. The COR will contact the CO for clarification of issues. Q5: Will we be responsible for batching machines for testing to stay in compliance? A5: We request that all annual surveys continue to be done in Sep 2016. If the vendor cannot complete all in Sep than we would have to agree on dates that allow the VA to ensure we have one survey per calendar year for each unit.
