Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with format in FAR subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotes are being requested and a written solicitation is included. Solicitation VA119-17-Q-0222 is issued as a Request For Quote (RFQ). The solicitation is being conducted under FAR part 13 Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94/95 effective January 19, 2017. This procurement is for Positive Airway Pressure Devices and Accessories for In Home Use (portable). The selected contractor shall be capable of providing an uninterrupted supply of items for distribution through the Medical Surgical Prime Vendor Program. The requirement is solicited as brand-name (see justification for details). The associated North American Industry Classification System (NAICS) code for this procurement is 339112 Surgical and Medical Equipment Manufacturing. The small business size standard is 1000 employees. This is a 100 %set-aside for Veteran-Owned Small Businesses (VOSB s). All quoters shall be verified and list in the Vendor Information Pages (VIP) database (www.VetBiz.gov) to be considered for award. See Attachment B Excel Price/Cost Schedule for list of line numbers, items, estimated quantiles for details. Note: Wage Determination does not apply. The provision at FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and an addendum is attached. Quoters shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications, if not available in System For Award Management. The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and an addendum is attached. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition and is attached. Quotes are due March 22, 2017 at 4:00 PM EST.
