Cases will continue at a rate of 8 to 10 surgeries per week and will increase as OR rooms are opened. Procurement of these stretchers is critical to the expansion of patient care. Date and place of delivery (FOB Destination): 04/15/2017 VA Health Care System 500 Foothill Drive Salt Lake City, UT 84148-0001 The provision at 52.212-1, Instructions to Offerors Commercial, applies to this acquisition. The provision at 52.212-2, Evaluation Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Lowest Price Technically Acceptable (LPTA) and contractor must be able to provide proof of being an authorized distributor for all quoted items, and any award made as a result of this request for quote, will be made on an All or Nothing Basis. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable, if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror, within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. When competition is not practicable, IAW FAR13.106-1(b), COs solicit from a single source for purchases not exceeding the simplified acquisition threshold. COs may solicit from one source if the CO determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). BRIEF DESCRIPTION OF SUPPLIES OR SERVICES REQUIRED AND THE INTENDED USE: The Salt Lake City Veterans Affairs Health Care System (SLCVAHCS) has a requirement for the procurement of Transport stretchers in the Same Day Surgery and Post Anesthesia Care Units. The current upgrading and opening of additional Operating Rooms will make it a necessity to increase the number of transport stretchers to ensure adequate amounts to provide care. 3. UNIQUE CHARACTERISTICS THAT LIMIT AVAILABILITY TO ONLY ONE BRAND, WITH THE REASON NO OTHER SUPPLIES OR SERVICES CAN BE USED: The Salt Lake City Veterans Affairs Health Care System has requested Brand Name only for these stretchers to maintain standardization throughout the Surgery Service Department, in the best interests of patient safety and handling. This also incorporates the same type of training across the board. 4. SUGGESTED SOURCE INFORMATION (Contractor Name, Address, City, State, Zip, and Telephone Number) : Brand Stryker- Multiple small business distributors 5. REASON THAT SUGGESTED SOURCE IS THE ONLY BRAND, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: Stryker USA is the exclusive U.S. distributor for the Prime Electric Big Wheel Stretcher 6. DESCRIPTION OF MARKET RESEARCH CONDUCTED AND RESULTS OR STATEMENT WHY IT WAS NOT CONDUCTED: This is a Brand Name only procurement for standardization; it is in the best interests of patient safety and handling to maintain standardization of the equipment. Market research has shown there are several SDVOSB vendors able to provide items regarding this requirement annotated in line 4 7.
