Description
STATEMENT OF WORK FOR QUARTERLY MAINTENANCE OF THE MILITARY AFFILIATE RADIO SYSTEM (MARS) ANTENNAS 1. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, parts, materials, vehicles, other items and services necessary to provide Quarterly Maintenance of six (6) Antennas located at the Military Affiliate Radio System (MARS) Station, Area X, Bldg. 1170 Beaver Road, Schofield Barracks on the Island of Oahu, Hawaii. 1.1. MAINTENANCE. The Contractor shall provide Quarterly Maintenance of towers, rotators, cables, and antennas at the MARS station on Schofield Barracks in accordance with all manufacturer/industry standards and Environmental Protection Agency (EPA) and Occupational Safety and Health Administration (OSHA) regulations. Maintenance should include but not limited to tower inspections, plumbing and tensioning of towers if applicable, checking and tightening of loose bolts, corrosion control, visual inspections of antennas and rotors, inspect all coax and connectors from antenna to building, replace coax or connectors if needed. 2. REPORT. Contractor shall provide a detailed service report including pictures reflecting work performed and results found that are not covered under this contract to the Contracting Officer Representative within 10 calendar days after the scheduled maintenance has been performed. 3. SERVICE PERFORMANCE SUMMARY. The Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. Current Log Periodic antennas and associated equipment on site and existing utilities are provided for the Contractor's use for this requirement. Description of Government furnished property is listed below. Detailed specifications are available for viewing through the contract COR. Antenna : "Skip Log Periodic Model: 7-10-30LP8 'Skip Log' Beam width: 65 deg. E and 75 deg. In H plane Power Handling: 3kW standard Boom length and diameter: 29.5 ft x 3" x.125 walls Maximum element length: 46 ft Turning radius: 28 ft Weight: 86 lbs Antenna Base : Hustler Model G7-136 constructed of high strength aluminum and fiberglass Antenna Positioner : M2 Antenna Systems, Inc. Model: OR-2800DC Input voltage: 110 / 220 VAC Motor voltage: 28-42 VACS Weight: 42 lbs * WARNING - OR-2800DC positioner will " NOT " work with RC2800 control boxes that supply AC power. Severe damage will result. Azimuth / programmable control unit with RS232 port: M2 Antenna Systems, Inc. Model: RC2800DC 6. SPECIAL PROVISIONS: 6.1. QUALITY CONTROL. Maintain a quality program to ensure services are performed in accordance with commonly accepted commercial/industry practices. 6.2. QUALITY ASSURANCE. The Government will evaluate Contractor's performance to ensure the requirements in the Service Performance Summary are met. 6.3. HOURS OF OPERATION. Principle Period of service operation will be performed during the normal duty hours of 7:00 a.m. to 4:00 p.m.; Monday through Friday, excluding Federal holidays. Contractor shall be permitted to work beyond these hours if desired at no additional charge. Request and coordination must be made with the Coordinator at least 2 working days prior to the actual work. 7. CONTRACTOR MANPOWER REPORTING. "The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the Contractor shall report ALL Contractor manpower (including subcontractor manpower) required for performance of this contract. The Contractor is required to completely fill in all the information in the format using the following web address. " https://contractormanpower.army.pentagon.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer, Contracting Officer's Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of Contractor employee entering data; (5) Estimated direct labor dollars paid this reporting period (including subcontractors); (6) Estimated direct labor dollars paid this reporting period (including subcontractors); (7) Total payments (including sub-contractors), (8) Predominant Federal Service Code (FSC) reflecting services provided by Contractor (and separate predominant FSC for each Sub-Contractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the Contractor with its UIC for the purposes of reporting this information); (11) Locations where Contractor and Sub-Contractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language; and (13) Number of Contractor and Sub-Contractor employees deployed in theater this reporting period (by country). As part of its submission, the Contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the contract period of performance not to exceed 12 months and must be reported within 30 days after the end of the contract performance period. Contractors may use a direct XML data transfer to the database server of ill in the fields on the website. The XML direct transfer is a format for transferring files from a Contractor's systems to the secure web site without the need for separate data entries for each required data element at the web sit. The specific formats for the XML direct transfer may be downloaded from the web site. *Note: Information from the secure web site is considered to be propriety in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data. For internal Army analysis, the reports and queries from the database shall not contain proprietary data. 8. OPSEC Requirement Per FAR Clause 52.242-5001 -- DOD LEVEL I ANTITERRORISM (AT) STANDARDS Use in all solicitations and contracts requiring contractor access to controlled Army posts. (a) Pursuant to Department of Defense Instruction Number 2000, "DoD Antiterrorism (AT) Standards," dated October 2, 2006, each contractor employee requiring access to a Federally-controlled installation, facility and/or Federally-controlled information system(s) shall complete Level I AT Awareness Training on an annual basis and receive a certificate of completion. The training is accessible from any computer and is available at https://atlevel1.dtic.mil/at/. The contractor is responsible for ensuring that all applicable employees have completed antiterrorism awareness training and shall certify that their workforce has completed the training through the submission of completion certificate (s) to the Contracting Officer and the Contracting Officer's Representative (if appointed) within five working days after contract award or prior to access to a Federally-controlled installation or information system. (b) In the event that the automated system at https://atlevel1.dtic.mil/at/ is not available (e.g., server problems), Level I AT Awareness Training can be provided by a qualified instructor. However, if the training is not completed online, the Level I AT Awareness Instructor qualification must be coordinated with the Installation Antiterrorism Officer (or Installation Security equivalent) and the resultant name(s) of approved instructors shall be provided the contracting officer or designee along with all associated cost or schedule impacts to the contract. Antiterrorism performance (Level 1 AT Awareness Training attendance and compliance) may be documented as a performance metric under the resultant contract, and be part of past performance information in support of future source selections. 9. Cell Phone Use Contractors operating vehicles on an Army post shall not use cell phones unless the vehicle is safely parked or unless they are using hands -free cellular device. The wearing of any other portable headphones, earphones or other listening devices (except for hands-free cellular devices) while operating a motor vehicle is prohibited. Civilians, contractors and visitors to the installation who fail to comply with this policy are subject to administrative action to include loss of driving privileges on post and removal from and/or denial of re-entry to this installation. 9.1. Restrictions on Driving with Bluetooth Device on Joint Base Pearl Harbor Hickam (JBPHH). In accordance with Navy HHQ regulations (CNICI 5530.14, CNIC Ashore Protection Program), cellular phones can be used on JBPHH when using hands free devices. Hands free devices include console/dash mounted or otherwise secured cellular phones with integrated features such as voice activation, speed dial, speakerphone or other similar technology for sending and receiving calls. There is no requirement that it be built into the vehicle. However, driver use of any portable, personal listening device worn inside the aural canal, around or covering the driver's ear (including wired or wireless earphones and headphones, Bluetooth® or similar technology) is not authorized on base while operating a motor vehicle, except for fire and emergency response or law enforcement/security purposes. Hearing aids or devices designed and required for hearing enhancement are exempt from this prohibition. Violation of the cell phone policy on JBPHH warrants a mandatory Joint Base Traffic Court appearance. It is up to the Traffic Court Administrator to determine what sanctions to impose. However, three points will be assessed against a driver's base driving record for violations of the cell phone policy. Bluetooth/hands free devices are authorized and required for cell phone use while driving on all US Army Garrison-Hawaii installations, Marine Corps Base Hawaii, Camp Smith, and Bellows Air Force Station.
