- Statement of Work The Centers for Disease Control and Prevention is seeking three U.S. States, through their EMS or public health agencies, or their bona fide agents, to provide EMS activation/response data the purpose of conducting occupational health surveillance analyses; specifically, to obtain from each state a limited dataset including all version 3 records contained in the state NEMSIS database, particularly including variables documenting the occurrence and nature of any fatalities, injuries or exposures to responding EMS crew members. See Statement of Work for complete details including evaluation data (section C.8). Remember, you must be registered at SAM.gov to submit a proposal. If you have questions about anything contained in the Statement of Work, please direct them to the Purchasing Agent at lml3@cdc.gov and they will be forwarded to the project officer/COR for response, which will be uploaded as an attachment to this announcement for use by all bidders. Questions should be submitted NO LATER than April 6, 2017, by noon (12:00 p.m. ET). Your proposal should be submitted by April 18, 2017, by close of business (4:30 p.m. EDT). Your proposal should include two separate documents, your technical proposal and your business proposal, and should be e-mailed to the Purchasing Agent at lml3@cdc.gov. In your technical proposal please address all segments of the Statement of Work and describe any methodology and procedures necessary. Your technical proposal need not be lengthy so long as it is complete. In your business proposal you should split the base year and the two option years into individual sheets. Please take into account any cost of living increases between option years since this will be a fixed-price award (no time and materials or cost plus fixed fee proposals will be accepted). Award shall be made to the three State agencies which submit the lowest bids technically acceptable, technical acceptability to be determined by the project officer/COR.
