Description

This is a combined synopsis/solicitation for a commercial item prepared in accordance with all parts of and format in Subpart 12.6, as supplemented with additional information included in this notice in conjunction with procedures in FAR Part 13. This announcement constitutes the only solicitation; a written solicitation will not be issued. This proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 13. Interested persons may identify their interest and capability to respond to the requirement or submit price quotations. This notice of intent is not a request for competitive price quotations. However, if there are two or more Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) who are capable of providing a quote at a fair and reasonable price, this acquisition will be set aside for SDVOSB/VOSB. All vendors shall be registered in the System for Award Management (SAM) prior to award. Registration information is located at www.sam.gov. SDVOBs/ VOSBs not listed in https://www.vip.vetbiz.gov/ are considered non-responsive and price quotations will not be evaluated. Award will be made to the responsive/responsible offeror who provides the technically acceptable product at the lowest price. NAICS code 339112/1000 employees applies. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, dated 19 Jan 2017. Only emailed requests received directly from the requester are acceptable. Email quotes to carol.lam@va.gov no later than the Wednesday, March 29, 2017. Receipt must be acknowledged on my desktop computer to be recognized as received. This is the only acceptable method for delivery of quotes. Please state if your company has any government contracts, i.e., GSA, SEWP, NAC FSS, BPA, or you plan on a submission under open market conditions. For any government contract, provide your contract number, terms and conditions; SIN the products fall under (if any), delivery schedule and contract expiration date.   Requirements are as follows: The VA Pacific Island Health Care System (VAPIHCS) is requesting to install additional Pyxis machines at the VAPIHCS Mental Health Service and the Eye Clinic. The additional Pyxis MedStation Systems will be integrated with the network software program currently being utilized. The existing main console is located within the ACC Pharmacy Service for the purpose of monitoring the par levels of medications stored in areas of use outside of the pharmacy and providing replacement stock in a timely manner to ensure patient care is not compromised. Currently, the VAPIHCS has the CareFusion Pyxis MedStation installed in each of the ACC Modules, Women s Health, Dental, and the Leeward and Kauai CBOC. Awaiting installation are the Kona, Hilo, Guam, Maui, American Samoa, Molokai CBOC s and the Dialysis Unit. MedStations will be installed once the Windows7 Platform has been installed. The SMART Remote Manager will be installed on all the refrigerators located outside of the Pharmacy to ensure controlled access to medications/vaccines stored in the refrigerator. The software and hardware currently in use is proprietary to CareFusion.
