Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI   53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Large business firms, to provide one (1) CORE Precision Guided Therapy System for the Clement J. Zablocki VAMC, 5000 W. National Avenue, Milwaukee, WI 53295, as described below. General Requirements: BRAND NAME (VOLCANO CORPORATION) OR EQUAL: One (1) Intravenous Ultrasound (IVUS) with Fractional Flow Reserve (FFR) System. Both IVUS and FFR capability on same system. 360 degree of view inside vessels. Not less than 30 frame per second image capture. Not less than 256 gray scale levels, with pre and post processing. Imaging and Analysis Tools include: FFR lesion assessment. Digital IVUS. High Resolution rotational IVUS. Colored flow assessment mapping. Automatic tissue classification. Software application allowing clinicians to record, review, measure and label images. Can be integrated with radiology display boom to show IVUS/FFR information on screen next to X-ray images. ECG integration option for gaited analysis. Full permanent integration with existing Siemens Artis Zee or Q.Zen Cardiac Catheterization Labs. System memory at least 8GB SD RAM. System storage at least 1TB 7200 RPM SATA. System archiving options: Local, DVD, DICOM Network. DICOM Services include DICOM Work list Management and DICOM Store for IVUS and FFR images. 19 Monitors. Separate touchscreen controller tablet, which allows users to operate the system from outside the sterile field. Medical Grade Printer. All associated accessory cabling components. Includes lifetime end user license agreement. One (1) Three-year Software Support Agreement. Entitles the VA to unlimited software updates and upgrades for 36 months AFTER the initial warranty period ends. One (1) Training of End Users. Up to two days on site training per month, and no fewer than two days on site per quarter (3 months). One (1) One Year Warranty. Deliverables or Deliver Schedule: The contractor shall deliver and install the product. After contacting facility point of contact at the VA, a specific time will be scheduled for the delivery of the product within 60 days of receipt of order. Product will be stored at Contractor s warehouse until the scheduled delivery/install is determined. Product will be delivered and installed during normal working hours, between 7:30am and 4:00pm, Monday through Friday, unless otherwise stipulated by the VA in the terms of the contract. Installation is part of the contract, the contractor will be responsible to remove all cardboard, refuse, debris, etc., from site on a daily basis. Documentation necessary includes: A statement acknowledgment/invoice and receipt of delivery. Upon completing installation of the system, the Contractor shall pretest the system. After pretesting the system, the Contractor shall notify the facility point of contact in writing that the system is ready for acceptance, and that it meets all requirements as specified. All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL OF THE FOLLOWING: Company name. Address. Point of contact (name/title/telephone number/e-mail address). DUNS number. Tax ID number. Indication of which business category (SDVOSB, VOSB, Small Business or Large Business) for which the organization qualifies. A capability statement that addresses the organizations qualifications and ability to provide the requirement depicted above, and proof (i.e. correspondence from manufacturer) of your firm being an authorized distributor for the manufacturer. Submit responses to the Contracting Officer, Thomas Chesak, at thomas.chesak@va.gov no later than 11:00 a.m. (Central Standard Time), 28 March 2017. VA69D-17-N-0796 CORE Precision Guided Therapy System for Milwaukee VAMC shall be referenced on all correspondence regarding this announcement. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. The NAICS Code 334510, Electromedical and Electrotherapeutic Apparatus Manufacturing, is applicable to this acquisition; the size standard is 1,250 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. The Government will use responses to this notice to make an appropriate acquisition decision. This is the only notice, no other copies of this notice will be provided.
