- The Veterans Health Administration, Network Contracting Office 4 is issuing a combined synopsis solicitation to procure a Blanket Purchase Agreement for a period of 5 Years. The service being solicited is for Health Care for Homeless Veterans (HCHV) servicing all three Delaware Counties for the Wilmington VA Medical Center (WVAMC) located in Wilmington, Delaware. The solicitation is for full and open competition. SCOPE: WVAMC is seeking contractor support to provide emergency residential placement with therapeutic and rehabilitation services for up to 16 eligible homeless Veterans at any given time who are in immediate placement into a safe environment in the New Castle, Kent, and Sussex counties. Bed placement is anticipated at 8 in the New Castle County, 4 in the Kent County and 4 in the Sussex County. Many of the Veterans have conditions that include serious mental illness, substance abuse disorders, and other medical conditions. The goal of the HCHV Program is to quickly stabilize the Veteran s medical conditions and provide placement into temporary housing while adhering to their complex needs with on-site treatment and case management services. The Contracted Emergency Residential Services (CERS) is to target homeless Veterans and prioritize the transitioning from the literal street homelessness. Emphasis is placed on the referral and placement into permanent housing or long term residential programs utilizing VA or community resources. General Qualifications: The Contracted Residential Services facility must be licensed to operate and located in the state of Delaware. The facility shall have experienced personnel on-site to provide the services required in the statement of work (SOW). The facility shall also pass an initial inspection conducted by the selected personnel from the Wilmington VA medical Center. The offeror shall meet all of the technical capabilities as listed in the SOW and submit all required documents per the solicitation to include the CERS Checklist, completed Solicitation Document as stated in Section E (Supplemental Instructions to offerors), and meet the technical capability factors listed in E.9, Evaluation Factors.
