Description
Solicitation Number: 0010972507 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95 (13 January 2017). This requirement is under the associated North American Industry Classification System (NAICS) Code 811219, Other Electronic and Precision Equipment Repair and Maintenance and the Small Business Size Standard $20.5M. The government contemplates award of a Firm-Fixed Price. Contractor shall provide a price to perform the requirements of the attached Performance Work Statement (PWS) using the attached price schedule. See Performance Work Statement for contract requirement details. INSTRUCTIONS AND INFORMATION TO OFFERORS: Place of performance shall be at Aberdeen Proving Ground, MD. The provision at FAR 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at FAR 52.212-2 - Evaluation -- Commercial Items, does not apply to this acquisition. Evaluation (a) The contract will be awarded to the Lowest Price Technically Acceptable (LPTA) offeror. The technical evaluation will be a determination based on information furnished by the vendor in the submitted proposals. The Government is not responsible for locating or securing any information which is not identified in the offer. The Government reserves the right to make an award without discussions. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. Performance Work Statement Toxicology Directorate Army Public Health Center Aberdeen Proving Ground, MD 21010 Coverslipper Maintenance Service Contract BACKGROUND: The Toxicology Directorate, U.S. Army Public Health Center (APHC), requires a maintenance contract for various items of laboratory equipment. This equipment supports our mission to provide pathology support for live animal toxicity studies for human health evaluations of new items of Army material. The instruments must be maintained in calibrated and operational status to support the requirements for Good Laboratory Practice compliance. INSTRUMENTATION: Thermo Scientific Clear Vue Coverslipper, Serial No. CV20151507. REQUIREMENTS: APHC requires the contractor to perform the following services: 1. Preventive Maintenance. The contractor shall perform one preventive maintenance visit annually to APHC in order to perform alignments and verify equipment meets operations specifications of the manufacturer. Contracted services are to be performed during on a date agreed mutually by APHC Contracting Officer's Representative (COR) and the contractor. This service must take place during normal duty hours (Monday through Friday, 0800 - 1600 hours EST/EDST). 2. Emergency Maintenance. The contractor shall perform up to 3 emergency visits annually to APHC. Contractor is required to respond during normal duty hours within 48 hours of the call being placed by the COR. a. Contractor's service representative shall report in person and notify the COR of his presence at the building (E2100) (telephone number 410-436-7169) or his representative (410-436-1305) prior to commencing services during normal duty hours (0800 until hours EST/EDST). Contractor's service representative shall not report or attempt service during non-duty hours. b. After performing Calibration/Verification/Certification services, the contractor's service representative will affix and/or update DD Form 2163 (Medical Equipment Verification/Certification). The contractor shall complete DD Form 2163 in accordance with the instructions provided in TB 38-750-2, or by the contractor. An equivalent form that indicates the following information provided by the service technician is acceptable: (1) Instrument Identification Number (2) Instrument Model Number (3) Instrument Serial Number (4) Frequency of service (e.g., annual) (5) Unit Identification Code (6) Certified by (7) Date completed (8) Date due c. For repair services, contractor will provide the COR an itemized list of services performed during the service call that includes any recommended corrective actions by the equipment operators. 3. Replacement part provision. Contractor will furnish non-consumable replacement parts at no additional cost to the government, including those discovered during the Preventive Maintenance visit, predicted to imminently fail due to fair wear and tear. 4. Operational and Safety Improvements. Contractor will furnish or install updates of software and/or equipment upgrades including Technical Updates recommended by the manufacturer. 5. Instrument Integration. Contractor will perform calibration and verification of any items of equipment installed during a service call. 6. Peripherals. Contractor will update or replace any peripheral equipment, to include but not limited to printers, monitors and keyboards, as deemed necessary by the contractor and agreed upon by the Contracting Officer and Division of Toxicologic Pathology Laboratory Manager or Chief, to resume normal function of the coverslipper.. 7. Telephone support. Unlimited live telephone technical support, 24 hours/day, 7 days/week. 8. Estimates. For services recommended by the contractor or user for the coverslipper or associated equipment but not covered under the maintenance contract, service may not begin without authorization from the government in the form of a funded Purchase Request or contract modification executed by the Contracting Officer. Installation Access and Security: All contractor employees, to include subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 30 calendar days after completion of training by all employees and subcontractor personnel. Contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity ATO). This local developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 30 calendar days of contract award and within 15 calendar days of new employees commencing performance with the results reported to the COR NLT 30 calendar days after contract award. For contractors that do not require CAC, but require access to a DoD facility or installation: Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations. Contractor Manpower Reporting: Cost for Providing Accounting for Contract Services - APG 5152.237-4900, Contractor is required to provide data on contractor manpower (including subcontractor manpower) for performance of this contract. The cost, if any, for providing this data shall be entered into the space provided at the appropriate CLIN. If no direct cost is associated with providing the data, enter "No Cost". Instructions, including the Contractor and Subcontractor User Guides, are available at the CMRA (Contractor Manpower Reporting Application) website at: https://cmra.army.mil As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractor's system to the secure web site without the need for separate data entries for each required data element at the web site. The specific formats for the XML direct transfer may be downloaded from the web site."
