**** Begin Word Document - 'FBO-DESCRIPTION' ***** This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in FAR Subpart 12.6- Streamline Procedures for Evaluation and Solicitation for Commercial Items, and is being conducted under FAR Subpart 13.5- Simplified Procedures for Certain Commercial Items apply. The solicitation number is VA256-16-R-0464 and is issued as a Request for Proposal (RFP) The proposed contract action is for a physician to be located in the Monroe, LA CBOC location, which the Government intends to solicit competitive offers. Interested persons may submit proposals, and all eligible proposals received prior to the response date listed on FBO will be considered by the Government. The requirement is a small business set aside. The NAICS code is 561320 and the size standard is $27.5 Million. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83).
