Description
Solicitation 2017-Q-66337 This is a combined synopsis/solicitation for simplified acquisition procedure items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 13.1, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is RFQ 2017-Q-66337 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention (CDC), National Personal Protective Technology Laboratory (NPPTL) of the National Institute for Occupational Safety and Health (NIOSH) intends to award a firm-fixed-price purchase order to a contractor for a Video Production - Title; The Necessity of Fit Testing; The When and the Why (Working Title). QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. This acquisition is set aside for small businesses under North American Industry Classification System (NAICS) code 512110 and small business size standard of $32.5M. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, the contractor must be registered in the System for Award Management (SAM) prior to award of the purchase order in accordance with FAR Clause 52.204-7. Quotes are due on 04/07/17, by 04:00 PM Eastern Time and must be sent to the attention of John Carolan via email at JCarolan@CDC.GOV. The Offeror must cite RFQ Number 2017-Q- 66337 in the email subject line. Evaluation and award of the purchase order will be performed in accordance with FAR 13.106-2 Evaluation of Quotations or Offers and FAR 13.106-3 Award and Documentation. No telephonic quotes will be processed. Please send any questions and quotes to John J. Carolan at JCarolan@cdc.gov. After evaluation of all proposals, the government intends to award to the lowest priced technically acceptable offer. Award will be via purchase order, as the independent Government estimate for this effort is significantly less than the simplified acquisition threshold. RFQ 2017-Q- 66337 is attached to this announcement, and offerors are cautioned to read all information and follow all instructions contained in the RFQ. Failure to submit all required documentation/information may result in a proposal being ineligible for award. Acceptance and FOB Point: FOB Destination It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website ttp://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95 (January 03, 2017). Include FAR 52.212-3, Offeror Representations and Certifications with Offer or indicate if it is available online at SAM.gov. Attachments: Request for Quotations 2017-Q-66337. NOTE - The award will be made on the basis of the lowest evaluated price technically acceptable (LPTA) offer.
