Description
Statement of Work Background: The VA Connecticut Veterans Health System maintains an annual service contract for the certification of a Best Theratronics Gammacell 3000 Blood Irradiator System in the Lab. Objective: This contract is to ensure safe and reliable performance while meeting all safety requirements, industry standards and NRC and TJC regulations. This shall be completed to ensure maximum uptime with minimal impact on patient care. Scope of Work: The services required include: Preventive Maintenance of unit by qualified personnel achieving performance that meets all specifications. Preventive Maintenance means the following: Inspection of equipment, checking all components safety features and accessories; Adjusting as necessary, source chamber or source drawer position; Checking, lubricating and, if necessary, adjusting drive mechanism; Ensuring proper operation of timing device; Checking and adjusting as necessary, all switches; Checking that radiation levels around equipment are normal and are within regulatory limits; Perform contamination wipe tests and provide copy to customer; Provide customer with inspection checklist in respect of above items; Checking power supply, drum rotation and operator control; Performance of Dose-mapping with reference to ISO/ASTM 51939 Practice for blood irradiation dosimetry if necessary Basic operator training for any new staff that have joined since the previous preventive maintenance visits; None this year. 3. Quality of Service Service engineers are trained and qualified to all relevant standards required to perform work on Irradiators 4. Costs Except as specifically provided herein, the contractor shall bear all labor, travel and material costs it incurs during the performance of preventive maintenance visit. 5. Warranty of Parts and Service All parts supplied during the contract coverage period are warranted for 90 days from supply of part. The service performed as specified to the customer during the preventive maintenance is warranted against workmanship for 90 days after completion of the preventive maintenance visit. 6. Scheduling North America has been divided into geographic regions, these regions are visited in pre-determined months, +/- one month, for a reduced rate from the standard billed service price. If you require a visit outside of these pre-determined months, the standard billed service rate will be charged. 7. Customer Responsibility Obligations under this preventive maintenance agreement,the customer remains responsible for the overall maintenance and safety of the equipment. Customers to provide their targeted central dose (with their purchase order) in order to complete the dose-mapping portion of the preventative maintenance visit. Safety Regulations: All applicable safety regulations shall be observed during all work of this contract. The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. The VA Radiation Safety Officer (RSO) shall have full authority to see that contractor fully conforms to all TJC, NRC agency requirements and any other jurisdiction deemed applicable. Scheduling and Hours of Work: All work in areas involved shall be scheduled with the Biomedical Equipment Support Specialist, John Klemenz, in Clinical Engineering (or designee) and the staff in the Lab area. Mr. Klemenz can be reached at x5813 or 8133. All contractors must first sign in at the Clinical Engineering Office in Building 1, 2nd floor Rm. 2-132.
