Description

This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Solicitation number VA262-17-Q-0641 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation. It is the offeror s responsibility to monitor this site for the release of amendments (if any). This notice and the incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95, effective January 19, 2017. This is a competitive solicitation; 100% set-aside for Small Business. The North American Industry Classification System (NAICS) code for this acquisition is 811219 Other Electronic and Precision Equipment Repair and Maintenance and the small business size standard is $20.5 million. Offeror must be registered in the System Award Management (SAM) at https://www.sam.gov, in order to be considered for award. See Section B.5 PRICE/COST SCHEDULE in Attachment I for the contract line item number, quantity, and unit of measures. Description of services: The Government is seeking for perform electrical substation maintenance and testing for VA Greater Los Angeles Healthcare System (VAGLAHS). As specified in the Statement of Work (SOW) in Attachment I Section B.3, the Contractor shall provide all resources necessary to accomplish all requirements in performing the maintenance and testing of electrical substation. Period of Performance: The work to be performed is anticipated to begin approximately on or before September 1, 2017 for a period of five (5) months. Place of Performance: VA Greater Los Angeles Healthcare System VA Hospitals West Los Angeles (11301 Wilshire Blvd, Los Angeles, CA 90073) Sepulveda Ambulatory Care Center (16111 Plummer Street, Los Angeles, CA 91343) Downtown Los Angeles Ambulatory Care Center (351 East Temple Street, Los Angeles, CA 90012) FOB Terms: Destination The provision at Federal Acquisition Regulation (FAR) 52.212-1, Instruction to Offerors - Commercial Items (Oct 2015), applies to this acquisition. See addendum to FAR 52.212-1 in Attachment I Section E (E.1) Solicitation Provisions. A similar provision at FAR 52.212-2, Evaluation Commercial Items is used. See evaluation procedures in Attachment I Section E (E.4). Offeror shall include a completed copy of FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with its offer. The clause at FAR 52.212 4, Contract Terms and Conditions --Commercial Items, applies to this acquisition and full text may be accessed electronically at http://farsite.hill.af.mil. See addendum to FAR 52.212-4 in Attachment I Section C (C.1). The clause at FAR 52.212 5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. See Attachment I Section C (C.7). Defense Priorities and Allocations System (DPAS) and assigned ratings are not applicable to this acquisition. Offer is due on April 14, 2017 at 2:00 PM PDT. All email attachments in response to this RFQ shall be in either Adobe or Microsoft Office format. Offeror is advised that the Government may be unable to receive other types of electronic files (e.g. compressed or zip files) or files in excess of ten (10) megabytes (MB). Upon receipt, a notification confirming receipt of email will be sent to the Offeror. It is the offeror s responsibility to ensure all required documents are included and completed as required by this solicitation.
