NORTHERN ARIZONA VA HEALTH CARE SYSTEM Prescott, AZ 86303 STATEMENT OF WORK Vital Sign Monitors with Rolling Stand Introduction: The Northern Arizona VA Healthcare System (NAVAHCS) is part of the Veterans Integrated Service Network (VISN 22) and serves a population of approximately 78,000 Veterans in a primary service area that includes six counties in North Central Arizona. NAVAHCS provides inpatient and outpatient care to Veterans in Prescott, Arizona and surrounding areas. The Bob Stump VA Medical Center, located in Prescott, Arizona, provides care to all Veterans from community-based outpatient clinics (Anthem, Flagstaff, Cottonwood, Kingman, and Lake Havasu City), and surrounding areas. NAVAHCS Mental Health department requires the purchase seven new vital sign monitors and same brand coordinating rolling stands for the Domiciliary. 2. Purpose: The Domiciliary s current vital signs monitors are past life-cycle and there are multiple issues with accuracy. The thermometer, blood pressure and O2 sat monitor results are often inaccurate. This requires having to swap machines with other machines to get decent readings. As a result the team is left with very few machines that actually function correctly. Our facilities CLC and PACT had to replace their vital sign monitor machines about a year ago for the same reason. The CLC and PACT had the same machines that the Domiciliary is currently having issues with. 3. Scope: Specifically, the domiciliary needs new vital sign machine monitors that functional at full capacity and can be relied upon to deliver accurate results. 4. Salient Characteristics of seven units/stands: Blood Pressure capability Mean Average Pulse (MAP) capability Temperature Pulse Pulse Oximeter Memory to collect vital signs from the previous several hours Ability to program blood pressure to be taken in intervals of every 5 minutes to an hour. Ability to take a recording of blood pressure. Same brand rolling stands to hold monitors. 4. Utility Requirements: Electricity 100-240 V AC 5. Assembly Requirements: Assembly completed by station 6. Access to Site for Delivery: Monday-Friday 7:30am-3pm except holidays.
