- COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS General Information Document Type: Combined Solicitation/Synopsis Solicitation Number: VA256-17-Q-0529 Posted Date: 04/04/2017 Original Response Date: Current Response Date: 04/18/2017 Product or Service Code: 6515 Set Aside (SDVOSB/VOSB): VOSB NAICS Code: 334510 ELECTROMEDICAL AND ELECTROTHERAPEUTIC APPARATUS MANUFACTURING Contracting Office The Department of Veterans Affairs, Alexandria VA Health Care System (AVAHCS) Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ)," The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-89 (7-14-2016). The associated North American Industrial Classification System (NAICS) code for this procurement is 334510, with a small business size standard of 1250. Interested Veteran Owned Small Business (VOSB) Quoter(s) must be listed and registered in Vetbiz.gov to be considered for Award. The Alexandria VA health Care System is seeking to purchase replacement HD Camera to capture and record the HD imagery quality of the mucosa and anatomical structures of the pharynx and larynx This is a BRAND NAME OR EQUAL requirement IAW FAR 52.211-6. All interested companies shall provide quotes for the following: Supplies Line Item Description Quantity Unit Price Total Price 0001 Panasonic 3CCD HD Camera Model 9214HD or equal The components needed include the camera body, camera control unit, power supply, cable for all connections within the system and output cables. Camera must be compatible and capable to interface the hardware/software utility settings originally set-up of the existing Pentax Digital Swallowing Workstation currently used at the location model (DSW13) #EE 40171 and (DSW11) #EE 38369. Camera is to replace the obsolete Pentax PSV4000. See Brand name or equal worksheet 2 EA 0002 5197-801 Zoom Coupler, 17.5mm 2 EA Delivery shall be provided NLT 30 days after receipt of order/award of the order. Delivery terms FOB destination. The contractor shall deliver line item(s) 0001 -0002 to: Overton Brooks VA Medical Center 510 E. Stoner Ave. Warehouse Shreveport, LA 71101 4295 Award shall be made to the quoter, whose quote meets the technical requirement to include delivery, and is the lowest price." The government will evaluate information based on the following evaluation criteria: (1) Technical capability of the item or equal item meeting the requirement to include delivery requirement and (2) lowest price". The following solicitation provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items FAR 52.212-3, Offerors Representations and Certifications Commercial Items Offerors must complete annual representations and certifications on-line at http://orca.bpn.gov in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required. The following contract clauses apply to this acquisition: FAR 52.211-6, BRAND NAME OR EQUAL 52.211-6 BRAND NAME OR EQUAL (AUG 1999) (a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation. (a) When a solicitation uses "brand name or equal" purchase descriptions, the contracting officer must include in the solicitation the clause at 852.211-73, Brand name or equal, and the provision set forth at FAR 52.214-21, Descriptive Literature. The contracting officer must review the requirements at FAR 14.202-5 when using the descriptive literature provision. All quotes shall be sent via e-mail to the renata.ott@va.gov. This is an open-market combined synopsis/solicitation for HD cameras as defined herein. The government intends to award a purchase order as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition." OR "The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:" Quoter shall list exception(s) and rationale for the exception(s). Submission shall be received not later than 04/18/2017; 12 PM via e-mail only. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
