Description: **************NO PHONE CALLS PLEASE********* This is a market survey for the conducting of asbestos surveys at 41 Air Route Surveillance Radar (ARSR) Shelter Sites, this work will include visiting each of the locations and conducting comprehensive asbestos survey of the interior and exterior of each facility; performing bulk sampling of materials suspected of containing asbestos; coordinating and scheduling all site evaluations with the Safety and Environmental Compliance Manager and SSC/facility manager; and reporting each asbestos survey in accordance with the attached draft statement of work. The responses to this market survey will be used for informational purposes only. This is not a screening information request or request for proposal of any kind. The FAA is not seeking or accepting proposals at this time. The FAA will not pay for any information received or costs incurred in preparing the response to the request for quote. Therefore, any cost associated with the request for quote submission is solely at the interested vendor 冱 expense. At this time the nature of the competition has not been determined. The FAA may decide to do a full and open competition or set aside all or part of the procurement for small business, service-disabled veteran-owned small businesses or eligible socially and economically disadvantaged businesses that are certified by the SMA for participation in the SBA 冱 8(a) Program. There may be multiple contracts awarded. The purpose of this market survey is to solicit statements of interest and capabilities from small businesses, service-disabled veteran-owned small businesses and 8(a) certified firms, capable of providing pulverizing, grading and paving and crack seal and seal coat, as described in the attached DRAFT specifications and drawings. This market survey is also being conducted in order to obtain the information necessary to determine whether adequate competition exists to set-aside the competition among small businesses, service-disabled veteran-owned small businesses or 8(a) certified firms. Interested firms are required to submit the attached Business Declaration form and a copy of their SBA 8(a) certification letter, if applicable. In order to make this determination, the FAA requires the following from interested vendors: Contractor must have prior experience in conducting or managing asbestos surveys, monitoring of abatement activities and performing abatement activities for the Federal Government in the following states: Alaska, Ohio, Idaho, Wisconsin, Arkansas, Washington, North Carolina, Kansas, Illinois, New Mexico, Texas, Colorado, Georgia, Arizona, Virginia, Nevada, Wyoming, Missouri, Nebraska, Pennsylvania, Tennessee, Mississippi, Kentucky, Iowa, and South Dakota. Contractor should provide previous project information for at least 2 projects within the last 5 years, submitting information including a list of qualifying contracts that include customer name, description of services, contract amount, contract date and term, and customer contact information. Contractor should provide description of specific experience in the following areas: a)Performance of projects that include asbestos testing, site surveys, abatement work, monitoring of abatement work. b)Performance of work that includes installation of floor covering, pipe insulation, wall covering, ceiling tiles, etc. c)Segregation, storage, transportation and disposal of asbestos containing materials in accordance with Federal, State, and local requirements at a site approved to accept asbestos waste materials. Contractor should submit a narrative that details the extent to which the company 冱 experience is similar to the work described in the statement of work. Contractor should identify which sites are of interest and/or whether the contractor can perform at all sites. Capabilities Statement should not exceed 10 (ten) pages in length and should be submitted via e-mail to the Contracting Officer. Respondents should identify any proprietary information in responses. Attached are the DRAFT Specifications and Drawings, and the FAA Business Declaration Form. Please respond by reviewing these items, submitting a completed, signed capability statement, and a filled-out and signed copy of the Declaration form (please submit electronically, email to address below).
