- Statement of Work The Centers for Disease Control and Prevention is seeking a company to conduct five focus groups with up to nine participants each that gather worker and employer input on knowledge of work-related muscoloskeletal disorders (WMSDs) resulting from manual material handling, conduct qualitative analysis of the data to extract common themes, and generate an executive summary of the focus group results for the National Center for Productive Aging and Work (NCPAW). See Statement of Work for complete details including special consideration (section C.7) which are prerequisites for successful evaluation. Remember, you must be registered at SAM.gov to submit a proposal. The period of performance shall be for one year starting at date of award. If you have questions about anything contained in the Statement of Work, please direct them to the Purchasing Agent at lml3@cdc.gov and they will be forwarded to the project officer/COR for response, which will be uploaded as an attachment to this announcement for use by all bidders. Questions should be submitted NO LATER than April 25, 2017, by noon (12:00 p.m. ET). Your proposal should be submitted by May 5, 2017, by close of business (4:30 p.m. EDT). Your proposal should include two separate documents, your technical proposal and your business proposal, and should be e-mailed to the Purchasing Agent at lml3@cdc.gov. In your technical proposal please address all segments of the Statement of Work and describe any methodology and procedures necessary. Your technical proposal need not be lengthy so long as it is complete. In your business proposal you should break out any labor rates for employees working on this project. This will be a fixed-price award (no time and materials or cost plus fixed fee proposals will be accepted). Award shall be to the company which submit the lowest bid technically acceptable, technical acceptability to be determined by the project officer/COR. At this time, the requirement is reserved for small business only. Large businesses who respond will not be considered. It is your responsibility to monitor FBO for changes to this announcement (posting of questions and answers, extension dates, etc.).
