- DOL WAGE DETERMINATION (REFERENCE ONLY) SAM REGISTRATION (REFERENCE ONLY) TECHNICAL EXHIBIT (LIST OF TREES) STAMEMENT OF WORK ADDENDUM SF 1449 The contractor shall provide all necessary labor, equipment, material, transportation, and supervision to apply pesticide to kill Gypsy moths in the hardwood trees on the campus ground at the U.S. Coast Academy, 15 Mohegan Ave. New London, CT. There are approximately 588 hardwood trees as listed on the Technical Exhibit (TE) situated on the campus of approximately 103 acres. All work must be done as specified in the attached Scope of Work (SOW) and TE. SITE VISIT: The USCG Academy recommends that any vendor interested in submitting their quote shall perform a site visit prior to submission of quote. Visits to the site may be coordinated with CWO3 Tom Huffman, 860-444-8554 or by email at Thomas.I.Huffman@uscg.mil. Failure to visit the site will not relieve the awarded contractor from the contracted work because of under/low bid.
