- Sources Sought Notice: Provide and Install Elevator Door Operator Boards Location: Ralph H. Johnson VA Medical Center, Charleston, SC. 29401 DESCRIPTION: This is a sources sought to determine the availability of potential sources having the skills and capabilities necessary to provide the SPECIFIED REQUIREMENT. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. Questions should be submitted by email to Michael.giffon@va.gov. Provide only the requested information below. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 238290. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought announcement. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. SPECIFIED REQUIREMENT: Network Contracting Office 7, Charleston, SC is seeking sources for a potential contractor to provide the following items/service: ITEM NO. DESCRIPTION QUANTITY UNIT ESTIMATED OR STOCK NO. UNIT COST -------------------------------------------------------------------------------- SERVICE': 1 THE CONTRACTOR SHALL PROVIDE DOOR OPERATOR BOARDS FOR ELEVATORS P3, P4 & P5 AS DESCRIBED IN THE STATEMENT OF WORK. 3 EA 2 THE CONTRACTOR SHALL INSTALL, ALIGN, AND TEST THE BOARDS IN ADDITION TO PROVIDING A ONE (1) YEAR PARTS AND LABOR WARRANTY. 3 JB REQUESTED INFORMATION: (1) STATEMENT OF CAPABILITY: Submit a brief description, ten (10) pages or less that demonstrates how your company has the supervision, facilities, labor and experience to provide the items/services required. Include past experience in providing these items to the VA, other Government (Federal or State) agency, or for a private facility. Include a letter from the Manufacturer stating your company is an authorized reseller, distributor, installer, and/or repairer of the requested items/services. Please specify your availability to start date and your address. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a VIP certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) (g) Include the DUNS number of your firm. (h) State whether your firm is registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ and/or the VetBiz Registry at http://vip.vetbiz.gov/. If not, please NOTE that any future solicitation could only be awarded to a contractor who is registered in SAM and to receive award based on VOSB or SDVOSB status you must be registered in the VetBiz Registry. If the requirement is available on your GSA, FSS or GWAC schedule include the schedule number and associated SINs. Your response should include both the STATEMENT OF CAPABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained above. Please place "VA247-17-N-0371 Elevator Door Operator Boards in the subject line of your email. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. See SOW below: GENERAL INFORMATION TITLE OF PROJECT: Obtain and install new door operator boards for P3, P4 and P5.. SCOPE OF WORK: The Contractor shall provide the parts and materials listed below to the Ralph H. Johnson VA Medical Center (RHJ VAMC) located at 109 Bee Street, Charleston, SC 29401, contact telephone (843)789-6783. In addition, the Contractor shall install all parts and materials, and provide a one (1) year parts and labor warranty that begins on the day the invoice is certified for payment. Local time is based on RHJVAMC location. BACKGROUND: The installed MOVFR boards in the P3, P4 and P5 elevators are obsolete, parts are no longer made or sold for the installed technology. The installed boards are no longer serviceable, and cannot be adjusted. New boards have been developed and are available. PERFORMANCE PERIOD: The contractor shall provide the materials and services under this Statement of Work (SOW) within 60 days after date of award unless otherwise directed by the Contracting Officer (CO). TYPE OF CONTRACT: Firm-Fixed price GENERAL REQUIREMENTS These specifications are a statement of the minimum level of work and services that are to be provided under this contract. They are not intended to be, nor shall they be construed as, limiting specifications or requirements. At a minimum, the contractor shall be required to take all steps and measures which would be taken by a prudent building owner to ensure that the materials supplied will provide satisfactory performance for the life of the product installed. The Contractor shall provide the following parts and materials: Quantity (qty) three (3) MOVFR replacement door operator boards, and any other materials necessary to properly install, align, and test the replacement boards. The Contractor shall install the replacement boards and materials in Elevators P3, P4 and P5 per the manufacturer guidelines, align, test and certify installation as necessary for any Federal, State, or local building or other code. The Contractor shall warranty the parts and labor for a period of one (1) year from the date that the invoice is certified for payment. CHANGES TO STATEMENT OF WORK Any changes to this Statement of Work (SOW) shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the Contractor through the actions of the parties other than the CO shall be borne by the Contractor. REPORTING REQUIREMENTS The Contractor shall obtain all necessary licenses and/or permits required to perform the work. In addition, the Contractor shall provide the CO with a copy of any licenses and/or permits obtained. TRAVEL All work is to be performed at the RHJ VAMC located at 109 Bee Street, Charleston, SC 29401, contact telephone (843)789-6783. The Contractor is responsible for any anticipated travel and per diem. GOVERNMENT RESPONSIBILITIES The VA shall grant the Contractor access permission to all areas of the RHJ VAMC necessary to provide the services required under this contract. Facilities will be available Monday through Friday from 07:00 to 17:30 local time exclusive of Federal Holidays unless CO authorizes work at other times in writing. CONTRACTOR EXPERIENCE REQUIREMENTS The Contractor shall provide evidence of successful installation of at least two (2) similar jobs to this requirement as proof of adequate experience. If subcontracted, the Contractor will provide evidence the Subcontractor possesses the required experience. CONFIDENTIALITY AND NON-DISCLOSURE It is agreed that: The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS The C&A requirements do not apply, and that a Security Accreditation Package is not required.
