- 1 Draft Statement of Work CB EQ040 OCT Analyzer Eastern Colorado Health Care System Denver, CO PURPOSE The purpose of this requirement is to deliver PKG EQ040 OCT Analyzer as part of the initial outfitting of the newly constructed VA Eastern Colorado Health Care System, Replacement Facility located at 1700 N. Wheeling Street, Aurora, CO 80045. SCOPE The Contractor shall deliver all items listed in section 2.5. All items must meet all salient characteristics defined in section 2.5. 2.3. The Contractor shall furnish all personnel, supplies, and equipment required for delivery. 2.5 SALIENT CHARACTERISTICS 2.5.1. Tomography, Optical Coherence (JSN U4256) Quantity: 1 Zeiss Cirrus HD-OCT Model Cirrus 5000 or equal Specifications/Salient Characteristics: Looking for exceptional visualization and precise measurement application, each one a necessary building block in making clinical decisions. Need to be able to discover, track, and analyze single pathological events from multiple points of view for greater certainty regarding next step in treatment. Needs to examine retinal detail at the pixel level for critical pathological elements, focus the signal lower in the scan window for assessment of the deeper choroidal tissue, and precisely target and capture the same tissue every time to ensure consistent comparison. Needs to perform reproductive analyses (algorithms measure and display layers for tissue targeting, segmentation, and producible measurements), comparative reference (diversified normative database of OHN, RNFL, ganglion cell/IRL, and macular thickness facilitate at-a-glance identification of anatomy outside normal limits), plus change measurement (automatically register with historical data, allowing for point-to-point measurements of change). Primary tool used for comprehensive glaucoma management. Technical Specification: Methodology - Spectral domain OCT Optical Source - Luminescent diode (SLD), 840 nm Scan speed 27K-68K A-Scans per Second A-scan 2.0 mm (in tissue), 1024 Axial resolution 5um (in tissue) Transverse resolution 15 um (in tissue) Tabletop model 90 degree orientation, with technician sitting 90 degrees from patient Spectral domain Optical Coherence Tomography (OCT), with Angiography In 1 single OCT scan, visualization of both micro vascular and structural information with a noninvasive dye free OCT scan Scan Speeds: 27,000 A-scans/sec; 68,000 A-scans/sec Approximate Dimensions: 26L x 18W x 21H (inches) Power Requirements Electrical Rating (115V) - Single Phase, 100 120V~ systems: 50/60Hz, 5A Accessories/Configuration/Computer/Software Options: Include Angio configuration Computer must run Windows 7, at minimum, if Windows Operating System Non-Windows Operating System will need to be confirmed by Office of Information and Technology (OI&T) Software shall require login to protect patient information Software shall allow image manipulation and comparison Software shall be able to send DICOM images and report to a Picture Archive Capture System (PACS) Vendor shall complete VA 6550 Appendix A form to outline the communication protocols as part of Bid Package 2.5.2. Table, Tomography (JSN U4256.A) Quantity: 1 Table for Zeiss Cirrus HD-OCT Model Cirrus 5000 or equal Specifications/Salient Characteristics Table to accommodate OCT model in section 2.5.1 - JSN U4256 Equal to item area dimensions may not exceed 39L x 22W (inches) Approximate Dimensions: 39" L x 22" W DELIVERY DELIVERY Delivery Timeframe: Immediately upon award. Acquisition of the above items is part of the initial outfitting and activation of the new VA medical center being constructed in Aurora, CO. Since construction is still occurring, delivery dates are approximate and will be coordinated with the successful contractor after award (see 2.6.1.7 Coordination). The Contractor shall contact the On Site Point of Contact to schedule a pre-delivery meeting to be conducted approximately 60 days prior to the initial award delivery date for verification of delivery dates. The Contractor may be required to adjust the delivery date from the date specified in the contract award document. A government-requested delivery delay up to 90 days after the delivery date specified in the contract award shall be at no additional cost to the government. Delivery Location: Contractor shall deliver all equipment to the Eastern Colorado Health Care System, 1055 Clermont Street Denver, CO 80020 Delivery Markings: Contractor shall deliver items in manufacturer's original sealed containers with manufacturer s name marked thereon. Deliveries shall be marked with the PO and contract number. Items on delivery vehicles not equipped with a Lift Gate will not be accepted. The delivery will have to be rescheduled and any delivery charges will be the responsibility of the Contractor. Delivery will be coordinated through the On Site Point of Contact. POCs for delivery are: TBD SITE CONDITIONS There shall be no smoking, eating, or drinking inside the construction site at any time. Delivery personnel must comply with all posted site safety requirements, to include the wearing of PPE. Minimum requirements are hard hat, over the ankle boots, reflective safety vest, eye protection and gloves. INSPECTION AND ACCEPTANCE 3.1. The Contractor shall conduct a joint inspection with the On Site Point of Contact upon completion of delivery. 3.2. In the event deficiencies are identified, the Contractor shall provide date when the identified deficiencies will be addressed if not addressed on the date of delivery. 3.3. The Contractor shall conduct a joint inspection with the On Site Point of Contact after addressing all deficiencies. 3.4. All deficiencies identified during joint inspections shall be fixed by the Contractor prior to government acceptance of the item. 3.5. Disputes shall be resolved by the Contracting Officer. Operation and Maintenance Manuals Binders - Quantity (2) each for items identified in section 2.5. 4.2. Digital Copies - Quantity (1) each for items identified in section 2.5. 4.3. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to On Site Point of Contact upon completion of delivery. 5. OPERATOR TRAINING 5.1. Contractor shall provide all training materials at time of delivery. 5.2 Contractor shall provide on-site two hour training sessions for eight surgery and ophthalmology clinical staff not to exceed two sessions. Training shall be scheduled on-shift, Monday Friday 8:00 AM 5:00 PM. 5.3 Vendor shall migrate current studies from original equipment which is Zeiss, Cirrus Midel HD OCT 4000. Patient data shall be accessible on newly installed unit to allow comparison and progression analysis. Data migration shall be completed at the time of installation. 6. PROTECTION OF PROPERTY 6.1. Contractor shall protect all items from damage during delivery. The Contractor shall take precaution against damage to the building(s), grounds, and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. 6.2. The Contractor shall perform an inspection of the building(s) and grounds with the On Site Point of Contact prior to commencing work. The Contractor shall be responsible for repairing or replacing any items, components, building(s), or grounds damaged due to negligence and/or actions taken by the Contractor or its employees. The source of all repairs beyond simple surface cleaning is the facility construction contractor (or appropriate subcontractor), so that building warranty is maintained. Concurrence from the VA Facilities Management POC and On Site Point of Contact is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 6.3. The Contractor shall be responsible for securing the items identified in section 2.5 and its own work tools and equipment during delivery. 7. SECURITY REQUIREMENTS 7.1. Security background checks are not required. 8. WARRANTY 8.1. The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery. 8.2. The warranties shall include all travel and shipping costs associated with any warranty repair. 9. INFORMATION SECURITY/PRIVACY REQUIREMENTS VA Maintenance/Installation Contracts 9.1. VA INFORMATION CUSTODIAL LANGUAGE Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor/subcontractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA. This clause expressly limits the contractor/subcontractor's rights to use data as described in Rights in Data - General, FAR 52.227-14(d) (1). 9.2. SECURITY INCIDENT INVESTIGATION The term security incident means an event that has, or could have, resulted in unauthorized access to, loss or damage to VA assets, or sensitive information, or an action that breaches VA security procedures. The contractor/subcontractor shall immediately notify the COTR and simultaneously, the designated ISO and Privacy Officer for the contract of any known or suspected security/privacy incidents, or any unauthorized disclosure of sensitive information, including that contained in system(s) to which the contractor/subcontractor has access. 9.3. LIQUIDATED DAMAGES FOR DATA BREACH a. Consistent with the requirements of 38 U.S.C. §5725, a contract may require access to sensitive personal information. If so, the contractor is liable to VA for liquidated damages in the event of a data breach or privacy incident involving any SPI the contractor/subcontractor processes or maintains under this contract. b. Based on the determinations of the independent risk analysis, the contractor shall be responsible for paying to the VA liquidated damages in the amount of $37.50 per affected individual to cover the cost of providing credit protection services to affected individuals consisting of the following: (1) Notification; (2) One year of credit monitoring services consisting of automatic daily monitoring of at least 3 relevant credit bureau reports; (3) Data breach analysis; (4) Fraud resolution services, including writing dispute letters, initiating fraud alerts and credit freezes, to assist affected individuals to bring matters to resolution; (5) One year of identity theft insurance with $20,00.00 coverage at $0 deductible; and (6) Necessary legal expenses the subjects may incur to repair falsified or damaged credit records, histories, or financial affairs. 9.4. TRAINING a. All contractor employees and subcontractor employees requiring access to VA information and VA information systems shall complete the following before being granted access to VA information and its systems: (1) Sign and acknowledge (either manually or electronically) understanding of and responsibilities for compliance with the Contractor Rules of Behavior, Appendix E relating to access to VA information and information systems; (2) Successfully complete the VA Privacy and Information Security Awareness and Rules of Behavior training and annually complete required privacy and security training; and (3) Successfully complete any additional information security or privacy training, as required for VA personnel with equivalent information system access. b. The contractor shall provide to the contracting officer and/or the COTR a copy of the training certificates and certification of signing the Contractor Rules of Behavior for each applicable employee within 1 week of the initiation of the contract and annually thereafter, as required. c. Failure to complete the mandatory annual training and sign the Rules of Behavior annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the contract until such time as the training and documents are complete.
