Description

SOURCES SOUGHT This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this is to gain knowledge of potential qualified sources and their size relative to NACIS 811490. Responses will be used by the Government to make appropriate acquisition decisions. Responses are not considered responses to any potential solicitation announcement. (DRAFT) STATEMENT OF WORK (SOW) HOCOMA LOKOMAT MODEL LS0175 (S/N 0176) 1. Contract Title. Repair of Hocoma Lokomat Model LS0175 (S/N 0176) Treadmill 2. Background. Michael E. DeBakey VAMC, Rehab and Extended care line located at 2002 Holcombe Blvd. Houston, TX 77030, requires repairs/maintenance for Hocoma Lokomat Model LS0175 (S/N 0176) Treadmill. Contractor shall furnish all labor, materials, parts, equipment, and tools required to repair Hocoma Lokomat Model Treadmill. 3. Scope. All repairs shall be performed by manufacturer licensed professionals using only original manufacturer equipment. Contractor shall provide documentation of at least three (3) past performance jobs of similar size and scope. Following the repair, the contractor shall demonstrate to the Contracting Officer Representative (COR) that the equipment runs properly according to manufacturer specified levels. The contract price shall be firm-fixed, all-inclusive not limited to specified products/equipment, installation, and setup/configuration. 4. Specific Tasks. Upon contract award, the contractor shall ship all parts to VA hospital at 2002 Holcombe Blvd. Houston, TX 77030 within seventy-two hours (72) hours with attention to David Myles located in BA-200 or option to bring parts with service technician/engineer at time of repair. Service shall be completed fifteen days (15 days) after recite of order. Contractor shall communicate with COR when the unit is ready for use. 5. Security Requirements. The contractor shall not have electronic connectivity to the devices and shall not have access to patient databases or receive screenshots with PHI present. Any electronic or magnetic data storage devices such as hard drives, etc. shall be turned into the ISO. Electronic or magnetic data storage devices shall not be removed from VA property. 6. Risk Control. The contractor shall coordinate with MEDVAMC staff to ensure no impact to patient care during maintenance. 7. Place of Performance. Repairs will be performed both at Rehab and Extended Care Line at Michael E. DeBakey VAMC Houston. 8. Delivery Schedule. Service shall be completed fifteen (15) days after receipt of order (ARO). All interested firms shall provide the following information: company name, address, point of contact, phone number, email address, capability statement, business size, DUNS and GSA contract (if any) pursuant to the NAICS code 811490. Responses to this notice shall be e-mailed to Orita.Jarvis@va.gov. Telephone responses will not be accepted. Responses shall be received in writing no later than Monday, April 10, 2017 at 10a.m. CST. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties shall respond to that solicitation announcement separately from the responses to this announcement. Responses are not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
