- Updated RFQ for SCBA Testing and Maintenance--reflects new due date for submission of proposals. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this requirement. Under this solicitation, a Request for Quotation (RFQ) is issued and is identified under solicitation number F1S2AA7040A001. Submit written quotations in response to this RFQ. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95 and Defense Federal Acquisition (DPN) 20161222. This acquisition is a 100% set-aside for Small Business. The associated NAICS code is 541380, with a size standard of $15.0 Million. The purpose of this solicitation is to acquire services to perform Flow Testing, equipment maintenance and repairs for 180 ea., Mine Safety Appliances (MSA) -- FireHawk M7, Self Contained Breathing Apparatus (SCBA) units in accordance with the manufacturer's recommendations and National Fire Protection Association (NFPA) 1852, Ch-7. Each unit includes: Harness Assembly, FireHawk M7 Responder Face Piece, Wireless M7 Heads-Up Display (HUD) with Auto Dim and Buddy Light, and Clear Command Wireless Commumication System (face mask) with breathing valve, HUD and bypass. Respondents to this solicitation are to provide a Service Technician who is qualified to work on MSA FireHawk M7 SCBA Units and who possesses proof of certification. The line item description for this requirement is as follows: CLIN 0001 - Perform on-site inspections, maintenance and annual flow testing for 180 MSA FireHawk M7 SCBA Units. Includes performing leak checks of the entire SCBA system utilizing the POSICheck system and performing warranty repairs and maintenance as recommended. On-site visit includes performing all required services identified in the Statement of Work (SOW) including SCBA system tests and associated maintenance checks and repairs. Respondents to this solicitation are to prepare a Quotation detailing the costs of providing all the required services for all 180 SCBA Units in accordance with the attached SOW and all applicable regulations and directives. The quotation must address all required services specified in the SOW. The respondent will be responsible for providing all test equipment, specialized tools, materials and labor necessary to perform all specified SCBA system tests and maintenance/repair actions. Also, the respondent shall provide a proposed start date to signify the first date a technician will be available to begin the specified work. The period of performance shall not exceed 30 days to complete requested services. EVALUATION OF QUOTATIONS/OFFERS: Quotes/offers will be evaluated IAW 13.106-2. Quotes will be evaluated according to lowest price. Also, respondents should be prepared to demonstrate (by providing appropriate documentation) that the technician performing the required services is certified to work on the MSA FireHawk M7 SCBA units. Quotes must be signed, dated and submitted via email, by 12 April 2017, 12:00 pm, Pacific Time to the following Government Representatives: john.wagner.15@us.af.mil, Phone 661-277-2546, and charles.richardson.16@us.af.mil, Phone 661-275-3248. LATE OFFERS: Offerors are reminded that an e-mail transmission of the quote/offer will not be recognized as a timely submission unless it is received by the Government prior to the closing date and time. Quotations/offers or modification of quotes/offers received at the address specified for the receipt of offers after the time specified for receipt of offers WILL NOT be considered (IAW FAR 52.212-1(f)). All contractors must be registered in the System for Award Management (SAM) in accordance with FAR 52.204-7 prior to any contract award. Please submit the following information with each quote: Cage Code, DUNS number, Tax Identification Number, Prompt Payment Terms, and date the offer expires. Solicitation Questions: All questions pertaining to this solicitation must be submitted No Later Than COB 28 March 2017. Responses to solicitation questions will be answered via Amendment to the FBO posting. This agreement incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
