Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs (Pathology & Laboratory) located at James A. Haley Veterans Hospital (JAHVAH) is seeking a Cost per Test which includes the following: a) equipment lease (free rental); b) reagents (at cost per consumption) and quality controls; c) maintenance & service warranty to include parts, operational hardware, software upgrade/updates, and user training. Contractor is required to provide delivery and installation of equipment at no additional charge; there also shall be no return shipping costs at end of contract. Response shall include manufacturer model# or part#, along with details on how such item(s) match or exceed the salient characteristics. Solicitation number VA248-17-Q-0429 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 & 2005-95, effective 01/20/2017. This RFQ is unrestricted. The associated North American Industry Classification System (NAICS) Code for this acquisition is 334516, size standard $15 Million. The Statement of Work (SOW) can be found at attachment A. Price Schedule can be found at attachment B. Provisions 52.212-1, Instructions to Offerors Commercial and 52.212-2, Evaluation Commercial Items apply to this acquisition. A Comparative Evaluation of the quotes received shall include the following factors: Price, Technical and Past Performance. Details of the evaluation can be found in the Addendum for FAR 52.212-2 at Attachment C. Provision 52.212-3, Offeror Representations and Certifications Commercial items (MAY 2014) applies and is included at Attachment D. Clause 52.212-4, Contract Terms and Conditions Commercial Items applies. Clause 52.212-5, Contract Terms and Conditions Required Implementing Statutes or Executive Orders Commercial Items (sub-clauses) applies and is included at attachment E. Equipment & Salient Characteristics can be found at attachment F. Any questions concerning this solicitation shall be submitted not later than 11:00 AM EST on April 14, 2017 via email to Leonora.simmons@va.gov. All quotes shall be submitted via email to Leonora Simmons not later than 11A.M. EST on April 21, 2017.
