Description
THIS IS A SOURCES SOUGHT NOTICE. (A Sources Sought is defined as not an actual bid or proposal solicitation; instead, it's a solicitation of interest. Think of a Sources Sought as market research being conducted by a government agency to determine what the capabilities and interests of the marketplace are).This is not a request for actual bid or proposal solicitation. A solicitation will not be issued at this time. The Department of Veterans Affairs, Phoenix VA Health Care System (PHOVAHCS) has a requirement for additional user licenses, phone training and voice recognition integration for one radiologist workstation. This is to help improvement their existing practices and processes by adding a new ABL 825 Blood Gas Analyzer. The intent of the procurement is to award a firm-fixed price contract. Phoenix AZ VA Health Care System (PHOVAHCS): 650 E. Indian School Rd. Phoenix AZ 85012 The applicable NAICS code is 334516 Analytical Laboratory Instrument Manufacturing. Small Business manufacturers for this NAICS code have a size standard of 1000 employees.   The resulting contract will be a firm fixed price order. The term of the contract is intended to be 30 calendar days ARO, with no options included. It is anticipated that a Request for Quotes will be issued in April 25, 2017. Award of a firm fixed price contract is contemplated by May 15, 2017. This notice is to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance of residential renovation projects (including references); (3) intentions of subcontracting program to include set-asides; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) documentation on company s bonding capacity and limitations; and (6) any other pertinent company documentation. The response date to this Sources Sought notice is April 21th, 2017 at 10:00 pm EST. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or sole-source procurement is appropriate. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. Statement of Work Equipment being ordered: ABL 825 Blood Gas Analyzer Please provide the following information clearly and precisely. Description of what the equipment being requested is: The ABL 825 Flex Series Analyzer with Auto Check with measured parameters include: pH, pCO2, thb, sO2, FO2Hb, FCOHb, FMetHb, Cl-, CA++,K+, Na+ and Glucose) is an essential laboratory device to report critical blood gas results rapidly in order for the physician to intervene quicker due to the critical reported results, especially in the ICU or Emergency room setting. (Auto Check are required consumable to operate equipment). Description of how the equipment being requested operates: The Respiratory Therapist that are assigned receive an order for a blood gas to be obtain from a patient that may critically compromised and may require life support measures. The Respiratory Therapist will introduce the patients blood gas sample into the Blood Gas Analyzer for measurement of the sample and verify the results per protocol and policies. The blood gas analyzer critical information will automatically after review by Respiratory Therapist made immediately available in VISTA-CPRS for the ordering physician to review. In addition, per policy, the Respiratory Therapist will telephone critical values to the ward/unit. Where will the equipment being requested be used: The blood gas analyzers will continue to be located in the ICU also on the 6th floor and the third blood gas analyzer is slated to be located in the new Emergency Department upon completion. The blood gas analyzer in the ICU and in the Emergency Department will actually serve as satellites to the main Blood Gas Laboratory that is located on the 6th floor. Who will be using the equipment being requested: The Respiratory Therapist that are assigned to the ICU or the Respiratory Therapist assigned to the new Emergency Department and the Respirator Therapist directly involved with out-patient homecare are the only authorized personal to operate the Blood gas Analyzers. Justification: The purchase of new blood gas analyzers ABL 825 will continue our ability to report critical blood gas results rapidly in order for the patient to benefit with the quicker intervention because of the reported results. The existing blood gas analyzers were acquired January 2003 with a recommended replacement date of January 28, 2013. BIO-MED has deemed it emergent as a cost effective method in repairs and parts, and to maintain our operation 24/7 of providing critical test results, to purchase new 825 ABL Blood Gas Analyzers replacements as soon as possible. The Phoenix VA will trading in the existing two ABL 825 Blood Gas Analyzer with this request and ordering one additional 825 Blood Gas Analyzer. The additional blood gas analyzer is to be located in the new Emergency Department that is currently under construction when completed. The purchase of the blood gas analyzers will continue to be located in the ICU also on the 6th floor and the third blood gas analyzer is slated to be located in the new Emergency Department upon completion. The blood gas analyzer in the ICU and in the Emergency Department will actually serve as satellites to the main Blood Gas Laboratory that is located on the 6th floor. The Respiratory Therapist that are assigned to the ICU or the Respiratory Therapist assigned to the new Emergency Department will allow our ability to continue to process the blood gas, and obtain the results without having to leave the ICU or Emergency department and report the critical results immediately. The existing and new replacements blood gas analyzers have the capacity of also reporting Glucose measurement for those patients with critically low blood pressures per facility standards. The Radiometer ABL825 blood gas analyzer is the top of the line of blood gas analyzer which will allow us to continue to have the advantage to ensure immediate results for our Veteran patients. This purchase agreement shall allow for a trade in allowance of $ for the existing ABL 825 blood gas analyzers and the purchase of the third ABL 825 blood gas analyzer to be made possible, to meet the growing demand of our new Emergency Department and serve our Veteran patients.
