Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, VA James J. Peters VA medical center is currently conducting a market survey for qualified firms capable of providing (Lease Equipment for Hudson Valley VA Medical Center) The vendor will be responsible for, but not limited to: STATEMENT OF WORK Title of Project: Cystoscopies contract lease Brand Name:Olympus 2. Scope of Work: Hudson Valley performs cyctoscopies weekly in the Urology department. The vendor shall provide urology cycto Nephr video scopes that are compatible with our current Endo works and future Endo Soft surgery software system. We need this lease to include 4 urology cystoscopes with a service agreement to ensure that if a scope is compromised or breaks a replacement scope will be received within 24 hours. Each cystoscope is used to do a procedure on patients. Each flexible cysto-nephro videoscope features improved insertion ability and patient comfort, thanks to the innovative EvolutionTip design. This versatile cystoscope is compatible with patented Narrow Band Imaging (NBI) technology that improves visibility of vascular structures. The large 2.2 mm working channel allows the use of all common endoscopic devices. Four built-in switches on the ergonomically designed control section allow the user to program frequently used functions for improved procedural efficiency. The vendor will verify all scopes receive in the contract. 3. Background: We have had a 3 year urology scope lease.. 4. Performance Period: 1 year lease with 2 contract option years 5. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 811219. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov).
