- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0444 and is issued as a request for quotation (RFQ) in accordance with FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 337215 Showcase, Partition, Shelving, and Locker Manufacturing and small business size standard is 500 employees. This is a total service disabled veteran owned small business set-aside. The requirement is for Custom medical supply storage units for use at the Louis Stokes Cleveland VA Medical Center. Delivery location is the Louis Stokes Cleveland VA Medical Center, 10701 East Boulevard, Cleveland, OH 44106-1702. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-95. Contract Title. Custom Medical Supply Storage Cabinets, and Custom Storage Solutions for Wade Park Operating Room and Sterile Processing Service. 2. Scope. Please see the drawings below they are included as a reference only and measurements should be taken during the site visit for accuracy. A site visit will be conducted at the Louis Stoke Cleveland VA Medical Center on Friday 04-14-2017 @ 1300 EST. Please contact Christopher Callihan at christopher.callihan@va.gov with question or concerns as well as to confirm your attendance. If no interested parties have contacted Christopher Callihan by 04-13-2017 @ 1200 EST the site visit will be canceled. Contractor shall verify all existing field conditions prior to providing a proposal, and is required to attend a scheduled pre-bid site inspection prior to submission of proposal. Contractor assumes the responsibility of surveying the existing conditions as part of their performance Christopher Callihan Contracting Officer Network Contract Office (NCO) 10 Email: christopher.callihan@va.gov VAMC Cleveland is requesting the following: Work for project OR 2540 is to be performed in Wade Park Operating Room Clean Support Room 2540 Work for project CR 115A is to be performed in Wade Park Sterile Processing Service (SPS), Scope Room, Room 116. Work shall be accomplished during evening and weekend hours in order to facilitate patient care. Phased installation will be required in order to keep the Operating Room and Sterile Processing Service (SPS) open for patient care during renovation. Operating Room is routinely closed except to emergency cases on weekends and holidays. Work performed during normal working hours and off-shift hours shall be coordinated with Assistant Chief, SPS and Nurse Manager, Operating Room Contractor shall verify all existing field conditions prior to providing a proposal, and is required to attend a scheduled pre-bid site inspection prior to submission of proposal. Contractor assumes the responsibility of surveying the existing conditions as part of their performance. Security Requirements The contractor will need to be granted physical access to the Operating Room Suite to install and test all equipment. There are no Information Security elements related to this procurement. At least 48 hour notice prior to requiring access shall be given by O.R. Nurse Manager or Assistant Chief of SPS to ensure access is not delayed. SPS and/or Biomedical personnel will be present during install. 5. Other Pertinent Information or Special Considerations. Contractor shall store products until Installation dates have been set between Engineering and SPS. 6. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A 7. Risk Control Contractor is responsible for following proper infection control practices while dealing with potentially contaminated equipment. Contractor also shall ensure that sterile practices are maintained with clear deviation from Clean and Dirty sides including but not limited to wearing sterile protective clothing, personal protection equipment including bunny suit, head covers, shoe covers, beard covers, and eye protection. In addition these items shall be changed every time upon entry into the department, and when going from clean to dirty sides. 8. Place of Performance. Louis Stokes VA Medical Center 10701 East Blvd Cleveland, Ohio 44106 9. Period of Performance. 90 Days ARO 10. Delivery Schedule. Installation and start-up shall be started no later than 60 days ARO and shall be completed no later 90 days ARO. EVALUATION CRITERIA/ SUBMISSION OF OFFER All Questions Shall be submitted in writing in response to the Solicitation and are due no later than Tuesday 18 April 2017 at 3:00 PM EST.
