This notice is intended strictly for market research. The purpose of this Sources Sought notice is to determine interest and capability of potential qualifications of Small Businesses Service Disabled Veteran-Owned firms relative to the North American Industry Classification Code (NAICS) 334517. The Department of Veteran s Affairs is seeking small business manufacturer or distributors of small business manufacturers, of functionally equivalent equipment to all the following: (1) New Cadwell Sierra Summit EMG Unit or Equal STATEMENT OF WORK (SOW) Electrograph Machine Northport VA Medical Center, Northport NY (632) Physical Medicine & Rehabilitation Service (113) 1. Introduction and Overview This SOW involves the replacement of the Cadwell Sierra Wave through purchase of Cadwell Sierra Summit system that will be used to evaluate nerve conduction velocities distal latencies in nerves, and amplitude of nerve signals to the muscles. The Sierra Wave, our current Electromyograph (EMG) was purchased in September 2010 and while still functional does not provide state-of-the-art documentation of changes in the electrical potential of muscles and nerves. The major difference is that Sierra Summit will allow more efficient analysis of wave forms, increased quality of the wave forms, and decrease patient discomfort due to decreased time of nerve stimulation. There are other Veteran Affairs Medical Centers who currently are using this system and this manufacturer is well aware of needs. Due to the high influx of patients and higher demands to eliminate wait time for patients needing electromyography testing another EMG machine is required. 1.1 Background New and improved clinical equipment are being developed every day to match the rising demands of an increased patient workload. Physical Medicine service staff across the nation relies on new technology to make care more precise, efficient, and safer for patients subsequently making their work less difficult. Advances in technology have impacted greatly on the diagnostic capabilities. 1.2 Scope of Work Our scope would be to acquire the newest generation Electrograph Machine. Objectives The primary objective for procurement of one Electrograph Machine must include: EMG, NCV, F/H, RNS, Blink, and RR Interval, SEP (Somatosensory Evoked Potentials) Small Form Factor Desktop PC Flat Panel LCD Color Wide Screen Monitor 24" USB Cable for Desktop PC 6' Microsoft Office Cart (Assembled) 1 - 2 Ch Amp Arm w/Stim, Amp & Needle holders Tripp-Lite Med Grade 4-plug Power Strip Scanner/Copier/Printer Electrode Kit Stimulator Probe USB Footswitch Temperature Probe Electronic Technical Manual Service Contract in addition to standard 1 year warranty Period of Performance- N/A Projected Cost To Be Determined 2. References- N/A 3. Requirements Equipment to be delivered/assembled upon receipt to medical center. Training of staff, minimum one person, to be performed on site. Statement of warranty of equipment, minimum one year. Statement of service included or for purchase: preventive maintenance; service and repair. 3.1 Tasks Deliver one Electrograph Machine to Department of Veterans Affairs, Northport VA Medical Center, Building 36 Warehouse,79 Middleville Road, Northport, NY 11768-2200. Assembly and validation of performance. Training of staff. **All highlighted yellow areas are an absolute must. Based on market research now, if you provide a quote later without these items, your quote will be considered incomplete and will not be allowed for competition. Please understand all items declared on this sources sought notice are required for this piece of medical equipment for the Department of Veterans Affairs use.** 3.2 End Results/Deliverables- N/A 3.2.1 List of Deliverables by Task- N/A 3.3 Schedules/Milestones Delivery of one Electrograph Machine within 30 days of purchase. Validation of performance within 30 days of delivery. Other Considerations- N/A 4. Progress/Compliance- N/A 5. Transmittal/Delivery/Accessibility The contractor shall provide [1] hard copy of each deliverable and one electronic version as well as user s manual. Notes- N/A CODES: Vendor shall comply with accepted manufacturer process, as well as current Occupational Safety and Health Administration, Labor Department, and New York codes, laws and regulations as applicable to this statement of work. THIS IS NOT A SOLICITATION AND DOES NOT OBLIGATE THE GOVERNMENT TO ISSUE A SOLICITATION. I am seeking all sources that can provide contract performance under this acquisition. Also, if this machine is proprietary in any way, please provide this documentation as well when responding to this sources sought to me via email. Do not provide quotes now. Please be advised that the Government will not pay for any information submitted under this announcement.
