Request for the Cleveland Veterans Affairs Medical Center for ThinPrep Processor Proposed solicitation number VA250-17-Q-0480 This is a combined synopsis/solicitation. This solicitation is issued as a Request for Quotation (RFQ), solicitation number VA250-17-Q-0480, in accordance with FAR Parts 12.603. This notice constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation document and incorporates provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, effective January 13, 2017. This procurement is set-aside 100% for Veteran Owned Small Businesses (VOSB) and only qualified Offerors may submit bids. The applicable North American Industry Classification System Code (NAICS) is 334516 Analytical Laboratory Instrument Manufacturing and the small business size standard is 1000 employees. Business size must be stated when submitting offer. If the item is on a FSS/GSA schedule awarded to the Offeror includes the schedule information on your offer. Awards resulting from the (RFQ) will be based on ability to meet specified technical request and the best value to the Department of Veterans Affairs giving consideration to: Technical, Delivery, and Price. The Government intends to award Firm-Fixed Price Set-Aside Requirement: Only verified VOSB firms are eligible to submit an offer or receive an award of a VA contract that is set-aside for VOSBs in accordance with VAAR Part 819. A non-verified vendor that submits a bid or proposal should be rejected as non-responsive or technically unacceptable as listing in VIP is not self-correctable as a minor informality as, for example, a listing in SAM. All other purported VOSBs must apply for and receive verified status in accordance with 38 CFR Part 74 and be listed in VIP prior to submitting a bid or offer on an acquisition conducted in accordance with VAAR Part 819. The VIP database will be checked both upon receipt of an offer and prior to award. New Equipment ONLY; NO remanufactured or "GRAY MARKET" items. All items must be covered by the manufacturer's warranty. No telephone requests for information will be accepted. Only emailed requests received directly from the Offeror are acceptable. It is the Offeror responsibility to ensure the quote is received. Quote must be good for at least 30 calendar days after close of this Combined Synopsis Solicitation. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Please provide a quote for the following item(s): ITEM INFORMATION ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Thin Prep Processor (Equal to or Better than) ThinPrep 2000 The equipment must meet the following salient characteristics: 1.Dimensions cannot be larger than 18 wide x 15 deep x 19.5 high 2.Must be able to process at least 20 samples per hour 3.Utilizes membrane technology 4.Automated, computerized system to prepare the needed slides 5.Utilizes the collection system currently in use in the Medical Center 6.The slide produced must be representative and reproducible or the original sample collected 7.System must have a mechanism to break up blood, mucus, and non-diagnostic debris 8.
