- 644-17-3-260-0441VA258-17-AP-2214 Performance Work Statement Toshiba Ultrasound Equipment Preventative Maintenance 14 April 2017 Phoenix Veterans Administration Health Care System (PVAHCS) 644-17-3-260-0441 Introduction: This requirement is for Preventative Maintenance Services on the Toshiba Xario 200 Ultrasound equipment, currently in use at the Carl T. Hayden Veterans Health Center located in Phoenix, Arizona. Background: The uptime of the Imaging systems used in the Surgical Clinic is critical to providing patient-centered care. The ultrasound must be maintained by a certified Contractor until Biomedical Engineering can be trained on servicing the modality. PVAHCS needs a Contractor who is trained and certified to maintain a Toshiba Xario 200 ultrasound. The Contractor must be knowledgeable in the hardware, software applications for the equipment listed. The Contractor must readily have access to parts that may be needed. The Contractor must be able to respond to emergency service calls within 4 hours to prevent further delay of patient care. The Contractor must be able to perform preventive maintenance to increase the dependability of the equipment. Required Services: Maintenance services shall include the following to meet agency needs: Coverage period: Monday-Friday, 8am-5pm LST, excluding federal holidays Uptime guarantee: 98% Phone response time: 30 minutes On-site response time: 4 hours Unlimited probe replacement coverage for multi-frequency linear probe PLU-1204BT.010 Printer coverage General parts coverage Parts delivery: next day Labor Travel Preventive maintenance: annual The scope of this requirement includes the equipment and attachments listed in subsection 3.2 Descriptions. The maintenance agreement shall include one base year, and four option years. Specific requirements are identified as follows: The Contractor shall notify either Biomedical Engineering or the Imaging Department of his/her intent to come on-site before arrival. The Contractor will check-in with the Biomedical Engineering Department upon arrival, before doing any work on-station. Maintain Toshiba Xario 200 ultrasound (S/N: 99B1453278) within coverage hours. The Contractor shall respond to phone calls within 30 minutes of being called. The Contractor shall come on-site within 4 hours of being dispatched. If the Contractor cannot respond within coverage hours, then he/she will respond as soon as coverage hours have restarted. Preventive shall be performed on an annual basis. The maintenance shall be scheduled with the owning department before arriving on station to work. The Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and confirms quality assurance tests results. The Contractor shall not remove any hard drives off station. All hard drives will be turned in to Biomedical Engineering. Security Requirements for Unclassified Information: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The Contractor may have VPN access as requirements dictate. The medical devices included in this contract capture and process ePHI. The Contractor may have access to ePHI while performing scheduled and unscheduled maintenance. Any and all ePHI will be stored long-term in either VistA Imaging or a server in the OIT server room that is maintained by Biomedical Engineering. A Business Associate Agreement does exist. Records Management Language for Contracts. The following standard items relate to records generated in executing the contract: Citations to pertinent laws, codes, and regulations such as 44 U.S.C. chapters 21, 29, 31, and 33; Freedom of Information Act 5 U.S.C. 552; Privacy Act 5. U.S.C. 552a; 36 CFR Part 1222 and Part 1228. The Contractor shall treat all deliverables under the contract as property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. The Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. The Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. The Contractor shall not create or maintain any records containing any government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information and all supporting documentation created as part of this contract. The Contractor must deliver sufficient technical documentation with all data deliverables to permit the Agency to use the data. The Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format, mode of transmission, or state of completion. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage, or alienation of Federal records is subject to the files and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. The Contractor is required to obtain the Contracting Officer s approval prior to engaging in any contractual relationship in support of this contract requiring the disclosure of information, documentary material, and/or records generated under or relating to this contract. The contractor is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. Risk Control: There will be no patients scheduled to be in the room with the Field Service Engineer. The room housing the equipment will be kept clean in accordance to the standards of the Environmental Management Service line. Performance Monitoring: The Surgical Department and Biomedical Engineering will oversee work done by the vendor and accept the quality of work as appropriate. The Contracting Officer Representative (COR) will be tasked with providing an evaluation of the technician assigned to conduct the annual PM of the equipment in addition to any unscheduled repairs in a Contractor Evaluation Form Report to the Budget Analyst who will compile the information in the Contractor Assessment Report. The Contractor Assessment Report will be forwarded on an annual basis to the Contracting Officer in providing guidance on the need for continued services in meeting option year requirements. The Government reserves the right to implement any additional monitoring of vendor performance as needed. Performance Monitoring: The Imaging Department and Biomedical Engineering will oversee work done by the vendor and accept the quality of work as appropriate. The Contracting Officer Representative (COR) will be tasked with providing an evaluation of the technician assigned to conduct the annual PM of the equipment in addition to any unscheduled repairs in a Contractor Evaluation Form Report to the Budget Analyst who will Government-Furnished Equipment (GFE) Government-Furnished Information (GFI): The uptime of the equipment is dependent on the facility s utilities, which are provided by the facility. Physical access to the equipment is also dependent on the facility. Other Pertinent Information or Special Considerations: Delivery of preventative maintenance shall be coordinated through the Administrative Offices of the Research Department located at: PVAHCS 650 E Indian School Road Phoenix, AZ 85012-1839 Preventative maintenance shall be conducted on the following equipment: SYSTEM S/N EE# LOCATION Toshiba Xario 200 99B1453278 89694 2227-1 Period of Performance: The period of performance shall be for one base plus four (4) option years. The Government may extend the term of the awarded base/option year by written notice to the Contractor within 30 days of annual expiration date, provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary (>60 day) notice does not commit the Government to an extension
