Description
The Department of Veterans Affairs, Network Contracting Office (NCO) 15 is conducting market research to help determine the availability and technical capabilities of qualified vendors who can provide FDG and NaF Radiopharmaceuticals to the Robert J. Dole VA Medical Center in Wichita, KS. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e. small/large), and DUNS Number. 2) Statement of Capability that demonstrates ability of providing the item in accordance with the attached Statement of Requirement (SOR) and past performance in providing this type of service. Include examples of prior completed Government contracts, references, and other related information. This notice is to assist the NCO 15 in determining sources only. This announcement is not a request for proposals or quotations. The Government is not committed to award a contract pursuant to this announcement. The Government will not pay for any costs incurred in the preparation or submission of information in response to this announcement. Questions and responses must be submitted no later than 10:00 a.m. (CDT) on April 19, 2017 to Yvonne Kittling at yvonne.kittling@va.gov. Any proprietary information should be clearly identified as "proprietary information. STATEMENT OR REQUIREMENT 1. General Description: Radiopharmaceutical preparation of doses and testing shall be performed at the Vendor s facility must be provided by a board-certified Radiopharmacist and delivered to the Robert J. Dole VA Cardiology Center. Personnel assigned by the Vendor to perform services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. Vendor shall hold a current unrevoked US Nuclear Regulatory Commission (USNRC) and/or Federal Drug and Administration (FDA) license to produce and distribute radiopharmaceuticals for human administration, and a license to receive, and dispose of radioactive waste. 2. Clinical Duties: Vendor s employees will be given escorted access to the Hot Lab that houses the Radiopharmaceuticals in the PET CT Center for deliveries and pick-ups. Delivery personnel will be bonded by the Vendor. 3. Administration: Vendor will bill on a monthly basis, in arrears. All services rendered will be itemized by patient name, date of service, radiopharmaceutical provided, and unit cost. 4. Security and confidentiality: Vendor shall be responsible for maintaining Automated Data Processing (ADP) Security. Vendor shall be responsible for safeguarding and maintaining patient information in the strictest confidence in accordance with the Privacy Act, HIPPA, and all other applicable state and federal laws. 5. Delivery: Vendor must be within the 50-mile radius of the Robert J. dole VA Medical Center or can deliver within a 1-hour timeframe. Vendor shall provide emergency deliveries as requested by the VA Nuclear Medicine Technician. Orders normally will be placed by 3:00 pm. Delivery shall be required by 6:00 am the following work day. Deliveries will be made Monday through Friday between the hours of 6:00 am and 4:00 pm. Due to the demands of the Medical Center s care requirement, there shall be no restriction to the number of orders and number of deliveries required per day placed by the VA Nuclear Medicine Technician s. The delivery order shall specify: patient name, contract item (s) and quantity, time (s) of delivery, and applicable VA order number. Orders can be placed orally by telephone and/or fax Transmission. 6. Delivery Location and Work Hours: Robert J. Dole VA Medical Center VA Cardiology Center 5500 E. Kellogg Wichita, KS 67218 The service covered by this contact shall be furnished by the Vendor as defined herein. The Vendor shall not be required, except in case of emergency, to furnish such services on a national holiday or during non-working hours as described below. Federal Holidays are as follows and include any other days specifically declared by the President of the United States to be a national holiday: New Year s Day Martin Luther King s Birthday President s Day Memorial Day Independence Day Labor Day Columbus Day Veteran s Day Thanksgiving Christmas Access to this location during non-work hours shall be obtained by contacting a Nuclear Medicine Technician at 316-651-3653 or 316-685-2221 ext. 53210. Deliveries will be supervised by the Nuclear Medicine Technician before beginning delivery operations. Vendor will contact the Nuclear Medicine Technician prior to delivery to arrange for delivery supervision. 7. Delivery Commitment: Time of delivery is mutually agreed upon at the time of receipt of telephone orders. 8. Person Authorized to Place Orders: The Contracting Officer s Representative (COR) and Certified Nuclear Medicine Technicians that are employed by the Robert J. Dole VA Medical Center are authorized to place orders under this contract. 9. Quality Control: Calibration time for each requested radiopharmaceutical will be provided by the Nuclear Medicine Technician at the time of order being placed. The quantity of radioactivity for supplied radiopharmaceuticals will not deviate more than -5% at the time of calibration, from the quantities specified. The expiration date and time shall be indicated and affixed to each product as noted below; in general: Ready-to-use doses: not shorter than one (1) hours. D. In the event that a quality control or material defect is suspected and/or detected by any Veterans Affairs Nuclear Medicine Technician, the Vendor will be requested to provide any consultation necessary to alleviate any said suspicion and/or defect by the following: Provide technical expertise in calibration; Provide professional examination of product; submit results and recommendations to the Contacting Officer; Make expedient replacement of any product suspected of being deficient. Vendor shall produce radiopharmaceuticals as described in the Bid/Price. Vendor shall make quality control records available to the VA Medical Center upon written request from the Contracting Officer.
