- This is a combined synopsis/solicitation for a commercial service. The West Los Angeles VA Medical Center is requesting Two (2), Three (3)-day workshops on Leading and Empowering Organizations prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA262-17-Q-0752 and is issued as an (RFQ) Request For Quotation, unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 611430 with a small business size standard of 11 million. This requirement is solicited as TOTAL SMALL BUSINESS SET- ASIDE and only qualified vendors may submit quotes. The VHA NCO 22 Long Beach, CA requires the following items: CLIN ITEM UNIT QUANTITY 0001 Two (2), Three (3)-day workshops on Leading and Empowering Organizations EA 1 DESCRIPTION/SPECIFICATIONS/WORK STATEMENT GENERAL PROCEDURE: The Contractor shall furnish all labor, materials, supplies, equipment, and transportation necessary to provide a workshop on empowering leaders & leading change in an organization for the Department of Veterans Affairs, Greater Los Angeles Healthcare System (VAGLAHS), located at 11301 Wilshire Blvd., Los Angeles, CA 90073. SPECIFICATIONS: Establishing a culture of continuous improvement is one of VA s primary area of focus. In order to believe that a change can occur requires leaders to feel empowered to help drive changes. Our current culture based on AES (All Employee Survey) data reveals a lack of trust that our leaders are capable of driving change. In addition, maintaining a competent workforce is key in improving the Veterans experience from the top down. A critical component in maintaining a competent workforce is sustained accountability as described by Secretary McDonald as his philosophy for conducting VHA business (Message from Secretary, Aug 28, 2014). Again, the AES scores reveal that we have much improving to do in the area of competency & building trust in the organization. Contractor will provide two, multiple day workshop sessions consisting of up to three 8-hour days. Handouts of presenter s material will be provided no later than two weeks prior to event date. Contractor will provide content outline(s) of the lecture material, CVs for all participating faculty and a Faculty Disclosure form for CEU purposes. Provide a course evaluation and share the results of the evaluation. During these training courses, novice and experienced leaders alike deepen their understanding of leadership & building trust within the organization. The workshop provides a conceptual framework for leadership, practical skills, and an opportunity to develop those skills. Participants also learn how to help their staff develop problem solving, relationship, and risk taking skills. The practical applications presented in these training courses should set it apart from other leadership programs. CONTRACTOR RESPONSIBILITIES: The Contractor shall provide the following, but not limited to: Two, training workshop sessions on dates to be mutually determined including: Handout Materials CEU Information If an event date is canceled or the scheduled consultant needs to be changed by contractor for any reason other than illness, death in the family, or weather related conditions, and alternative dates or a suitable substitute agreeable to the client is not found, contractor will refund applicable moneys already received from the client for canceled date. GOVERNMENT RESPONSIBILITIES: The Government shall not furnish any items/materials/property for this procurement. The Government shall coordinate all on-site training. Identify and provide meeting/training class rooms. Announce program, recruit candidates, processing all records and program information pertinent to program evaluation, and continuing education. CHANGES: The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. VA POLICY: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. Name: Title: Address: City, State, Zip: Phone No.: Fax No.: Email: The POC shall be available by telephone Monday through Friday, between 8:00 a.m. to 4:30 p.m. excluding national holidays. CONTRACTING OFFICER REPRESENTATIVE (COR): A representative from the VAGLAHS shall be the COR(s) for this contract. A Delegation of Authority letter shall be forwarded to the using service and the Contractor after agreement has been signed, identifying the individual(s) as the COR(s). No service shall be provided without the approval of the COR or his/her designee of this contract. The Contractor shall not accept any instructions issued by any other person(s) other than the Contracting Officer or his/her delegated representative(s) acting within the limits of his/her authority. CONTRACTOR PERSONNEL: All Contractor personnel assigned to the performance of this contract shall wear a Contractor-provided uniform and/or badge that clearly identify the individual as a Contractor. The parties agree that the Contractor personnel working under this contract shall not be considered VA employees for any purpose and shall be considered employees of the Contractor. Contractor personnel shall be required to speak fluently and comprehend the English language. The Contractor shall ensure that the staff assigned shall be cognizant of the VA s sexual harassment and drug-free workplace policies. INSURANCE: The Contractor shall be required to comply with Federal and State Workers Compensation and liability insurance. Reference FAR clause 52.228-5-Insurance and Subpart 28.307-2-Liability. Within 15 days of contract award, the Contractor shall furnish to the Contracting Officer certification from the insurance company indicating that the coverage has been obtained and that it may not be changed or cancelled without guaranteed thirty (30) day notice to the Contracting Officer. New certifications shall be furnished at least 15 days prior to the expiration date of the current insurance policy. SAFETY AND FIRE PREVENTION All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering the VAGLAHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. PARKING POLICY It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. IDENTIFICATION BADGES All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. SMOKING POLICY Smoking is not permitted within or around the VA Healthcare System facilities, except in designated areas. NORMAL WORKING HOURS: Normal working hours are defined as 8:00 a.m. to 4:30 p.m., Monday through Friday, except weekends and national holidays. NATIONAL HOLIDAYS: Any holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government. The Contractor is not required to provide service on the following U.S. Government holidays nor shall the Contractor be paid for these days: New Year s Day January 1 Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4 Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25 When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. Also, included, would be any other day specifically declared by the President of the United States to be national holiday.
