Attachment 3 Contractor Information Attachment 1 Performance Work Statement Attachment 2 DOL Wage Determination This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, supplemented with additional information included in this notice. This announcement constitutes one (1) solicitation which will result in one (1) Firm Fixed Price award for providing Leadership Development Course for the Special Tactics Training Squadron, Hurlburt Field Fl. Anticipated awards will be the result of Small Business set-aside. Proposals are being requested and written solicitations will not be issued. Solicitation Number FA4417-17-T-0009 is hereby issued as a Request for Quote. This combined synopsis/solicitation and incorporated provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-95, effective 19 Jan 2017. The North American Industry Classification System (NAICS) code for this project is 611620 and Standard Industrial Classification (SIC) code U013. The Size Standard for NAICS 611620 is $7.5M. This acquisition is solicited as a 100% set-aside for Small Business Concerns The contractors shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide Leadership Development Course training. Leadership Development Course - Troop Leading (LDC-TL): The purpose of LDC-TL is to facilitate combat arms leadership professional development for new and seasoned leaders. This three (3) week (15 training days) course of instruction shall provide academic and practical instruction covering small unit infantry (squad/troop) roles and responsibilities, troop leading procedures, basic patrol functions and battle drills, and combat orders. This objective shall be accomplished by classroom lecture and field training exercises (FTX) (small unit patrols) informed by historical and current intel/scenarios. Students will execute different leadership duties during each of the patrol iterations. The desired course end-state(s) are: All students participate in each patrol iteration. All students receive individualized verbal and written counsel on performance and are able to articulate their leadership strengths and weaknesses. All students practice Critical Thinking and Tactical Decision Making. All students learn and practice Tactical Mission Planning from Warning Order (WARNO) to Execution and post mission debrief. LDC-TL should provide students baseline proficiency in small unit tactics, troop leading procedures, and leadership. The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: 52.204-7 -- System for Award Management. (Jul 2013) (a) Definitions. As used in this provision- "Data Universal Numbering System (DUNS) number" means the 9-digit number assigned by Dun and Bradstreet, Inc. (D&B) to identify unique business entities. "Data Universal Numbering System+4 (DUNS+4) number" means the DUNS number means the number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this 4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional System for Award Management records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at Subpart 32.11) for the same concern. "Registered in the System for Award Management (SAM) database" means that- (1) The Offeror has entered all mandatory information, including the DUNS number or the DUNS+4 number, the Contractor and Government Entity (CAGE) code, as well as data required by the Federal Funding Accountability and Transparency Act of 2006 (see Subpart 4.14), into the SAM database; and (2) The offeror has completed the Core, Assertions, and Representations and Certification, and Points of contact sections of the registration in the SAM database; (3) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification Number (TIN) with the Internal Revenue Service (IRS). The Offeror will be required to provide consent for TIN validation to the Government as a part of the SAM registration process. (4) The Government has marked the record "Active". (b) (1) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. (2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "DUNS" or "DUNS+4" followed by the DUNS or DUNS+4 number that identifies the offeror's name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the SAM database. (c) If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. (1) An offeror may obtain a DUNS number- (i) Via the internet at http://fedgov.dnb.com/webform or if the offeror does not have internet access, it may call Dun and Bradstreet at 1-866-705-5711 if located within the United States; or (ii) If located outside the United States, by contacting the local Dun and Bradstreet office. (d) If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. (e) Processing time, which normally takes 48 hours, should be taken into consideration when registering. Offerors who are not registered should consider applying for registration immediately upon receipt of this solicitation. All offers shall be complete, clearly presented, and contain sufficient detail for effective evaluation as detailed in Addendum 52.212-2 of this solicitation. Offers shall be neat, indexed (cross-indexed as appropriate) and assembled in an orderly manner. Elaborate artwork, expensive visual, and other presentation aids are neither necessary nor desired. Offers shall only contain information that is relevant to this solicitation. C. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts; Part I - Price Proposal, and Part II - Technical proposal. D. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price. C. Specific Instructions: 1. PART I - PRICE PROPOSAL (a) Complete pricing proposal above to this solicitation. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The SOLICITATION constitutes the model contract. (b) Insert proposed unit and extended prices in the pricing proposal above. The extended amount must equal the whole dollar unit price multiplied by the number of units. The proposal(s) must be submitted for an entire period of performance. PERIOD OF PERFORMANCE: (Estimated 22 May through 1 Sep 2017) (c) Complete the necessary fill-ins and certifications in provisions. The provisions FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, shall be returned along with the proposal. 2. PART II - TECHNICAL PROPOSAL - Limited to no more than 10 pages. Technical proposal shall include offerors technical approach to the requirements stated in the solicitations Performance Work Statement. The following factors shall be used to evaluate offers: (1) TECHNICAL, and PRICE with Technical being approximately equal to Price when being evaluated. Initially, offers shall be ranked according to price. An offeror's proposed prices will be determined by multiplying the quantities identified in the Pricing provided above by the unit price for each item to confirm the extended amount for each. After being ranked according to price in ascending order, the proposal will be provided to the technical team for technical evaluation. If the lowest priced proposal is deemed technically acceptable, evaluations will conclude at this point. If it is not deemed technically acceptable the next lowest priced proposal will be evaluated. This process will continue until the Government has determined a proposal to be technically acceptable. (b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The Government will award a Firm Fixed Price Purchase Order on the factors above. The offeror shall submit the "Contractor Information" (attachment 3) and their technical approach based on this requirement Performance Work Statement. Price: Provide cost per 3 week course. Technical acceptability will be determined by the following: Program of Instruction for the course: Program of Instruction will be rated acceptable if it meets all the requirements of paragraphs below. The Program of Instruction clearly demonstrates a reasonable approach to managing and staffing the contract based on the requirements of the PWS. The offeror shall demonstrate their understanding of the course requirement(s) in the submission of a sample POI that (1) addresses current training offered and (2) draft course that include all elements required for LDC-TL course. Ready state draft POI for LDC-TL will be required for the course starting on 22 May 2017. Current training offered can be tailored to encompass elements required for the courses IAW the PWS. In addition, the offeror shall list the names and number of qualified personnel proposed to conduct the course. The format for the sample POIs must demonstrate: a breakdown of each day for each individual course including a detailed concept of operations, Government equipment, facility, and personnel support requests (if any); notional training timeline (event start / stop time); and overview of learning objectives by event IAW paragraph 1.4 and all subsequent paragraphs, Appendix F, and Appendix G in the PWS. The Offeror's proposed personnel meet the Personnel Qualifications IAW PWS paragraphs 1.5-1.5.21. Offerors shall submit resumes for the offeror's proposed personnel who will conduct each of the proposed courses and identify all qualifications in the resumes. Note: letter(s) of intent / proof of employment must be provided for each proposed instructor. In addition to the qualifications in paragraphs 1.5-1.5.21, Offerors' proposed personnel shall possess (1) Tier 1 Special Operations Operator Training Course (OTC) graduate, (2) At least two (2) years' experience within USSOCOM Special Mission Unit, and (3) must have held one of the qualified positions for each course they will provide training for as outlined specifically in PWS paragraphs 1.5.1-1.5.5. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
