- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation VA101V-17-Q-0175 is issued as a request for quote (RFQ). The solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-95, Dated 1/19/2017. The solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Business. The applicable NAICS Code for this solicitation is 484210 and the Small Business Size limitation is $27.5M. This solicitation is set-aside exclusively for Service-Disabled Veteran-Owned Small Business (SDVOSB) concerns in accordance with Public Law 109-461, Veterans First Program. Only offers from SDVOSB concerns that are registered and verified in the Center for Verification and Evaluation (CVE) Vendor Information Pages (VIP) website at https://www.vip.vetbiz.gov/ at the time date set for receipt of offers shall be considered for award. Offers from SDVOSB concerns that are not registered shall be rejected as non-responsive and shall not be considered for award. The contractor shall provide all resources necessary to accomplish the deliverables as described in the Statement of Work below (section vi). NOTE: Also see attached questions and answers. Please provide pricing for Line Items 0001: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Contractor shall provide all labor, materials, transportation, and equipment necessary for the removal and transport of a high-density file system per the Statement of Work. 1.00 JB __________________ __________________ Description of Requirement Statement of Work Background: The San Juan Regional Office is currently hiring personnel to establish a National Call Center (NCC). The purpose of the NCC is to provide services to veterans and beneficiaries by phone. The NCC will be located in a space that is currently occupied by a High Density File System (HDF). In order to make the space available for the NCC, the HDF must be dismantled and relocated. Type of Contract: Firm-Fixed-Price Performance Requirements The Contractor shall provide all labor, materials, transportation, and equipment necessary to remove one side of the High Density File System located on the first floor of the San Juan Regional Benefits Office, 50 Carr 165, Guaynabo, PR 00968. Work includes the dismantling of twenty-eight (28) shelves of the HDF in an organized and protective manner; tagging of the system s electrical system so that it can be easily re-installed at a future date; palletizing of the parts for safe transportation and storage; and transportation and delivery to a warehouse in Carolina, PR (exact location to be provided at a later date). The system must be disassembled in a manner that would allow the VA to re-use the equipment. 4. System Specifications Manufacturer: Spacesaver Model: High Density Eclipse Powered Storage System (custom built) 24 carriages; file size shelves; 85,000 linear inches of storage capacity. The part of the system planned to be moved covers an approximate area of: 1,500 sqft (the full system covers ~ 3,300 sqft, with the hallway in between) This product was installed in sept 2010, for which its warranty is concluded. 5. Security Requirements: Current rules and regulations applicable to the premises, where the work shall be performed, shall apply to the contractor and its employees while working on the premises. These regulations include but are not limited to, escort during work by VBA officials, presenting valid identification for entrance, smoking restriction and any safety procedures as outlined in the site regulations. 6. Safety Regulations: All work performed under this contract shall comply with health and safety regulations to include but not limited to Occupational Safety and Health Administration (OSHA) and other pertinent state and Federal requirements. Performance Period: The contractor is required to complete all work within ten (10) calendar days after receipt of award. Award is anticipated on or around May 1, 2017. Please report to the security guard and they will escort you to the necessary office of Carmen Albino and/or Benjamin Bruno. This will be the only opportunity for potential offerors to visit the site. All potential offerors, subcontractors, and suppliers are strongly encouraged to attend this site visit. Submission of Offers: Offers are due no later than May 2, 2017 by 2:00 PM EST. Offers shall be submitted electronically to Heather.Hampton@va.gov. The email subject line must contain the following: Quote in Response to VA101V-17-Q-0175, Dismantle and Move HDF. Offers shall be based on the requirements of the solicitation, and must contain the following: Company name, address, DUNS Number, tax identification number (TIN), and point of contact with phone number, and e-mail address. A completed price schedule in the format as provided in v ; Acknowledgement of any/all amendments; Solicitation Questions: Questions of a technical nature shall be submitted to the Contracting Officer in writing via e-mail. Oral questions of a technical nature are not acceptable due to the possibility of misunderstanding or misinterpretation. The cut-off date and time for receipt of questions is April 27, 2017 @ 2:00 PM EST. Questions received after this date and time may not be answered. Questions will be answered in a formal amendment to the solicitation so all interested parties can see the answers. Amendments: Amendments to this solicitation will be posted at http://www.fedbizopps.gov(FBO). Paper copies of the amendments will NOT be individually mailed. By registering to receive notifications on FBO, offerors will be notified by email of any new amendments that have been issued. No other notification of amendments will be provided. Potential Offerors are advised that they are responsible for obtaining and acknowledging any amendments to the solicitation. Failure to acknowledge an amendment may result in your offer being considered unacceptable. Basis for Award: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price as the only factor considered.
