Page 1 of 3 COMBINED SYNOPSIS/SOLICITATION (4) Bariatric ENT Exam Chairs SYRACUSE VA MEDICAL CENTER This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. (ii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number VA242-17-Q-0532. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This procurement is being issued as 100% set-aside for SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES (SDVOSB). It is the Government s intent to issue a firm fixed price, single award as a result of this solicitation. The North American Industry Classification System (NAICS) code is 339113, Hospital furniture, specialized (e.g., hospital beds, operating room furniture), with a small business size standard of 750 employees. This solicitation is 100% set-aside for SDVOSBs. (v) The Contractor shall provide all parts and labor necessary to deliver four (4) Bariatric ENT Exam Chairs that meet the brand name or equal specifications as identified below. The requested products shall be delivered in new and unused/unopened condition. (vi) Description of requirement: The Syracuse VA Medical Centers has a requirement for: Four (4) ENT Bariatric Exam Chairs that possess the following characteristics of Brand Name or Equal to the 423 Dual Power Exam Chair with Flip-up Footrest and oval Articulating Headrest as manufactured by Medical Technology Industries Inc.: - Capability to swivel at least 340 degrees with lock/unlock feature Capability to lower to at least 19 to allow for wheel chair transfer Lift base of at least 11.5 vertical rise capability Capability to accommodate up to 650lb patient weight Includes at least 1 IEC-320 outlet plugs on each chair for peripheral equipment Tight small knob lock system allowing for medical personnel adjustment Dual chair controls; controls available on both sides of chair 24 volt, Low voltage motor Flip up lock down arm rests Patient footrest At least (2) user settable program positions with auto return, stop/cancel feature Articulating Patient Headrest Quick/Emergency release function LED Exam light with plug accessory, attachable to chair (no access cords from wall to chair) -Aids in reducing trip hazards- No corded hand/foot controls (vii) EVALUATION: Quotes will be evaluated based upon lowest price technically acceptable. Technical acceptability is based upon the ability to meet the above specifications and past performance.
