Description
This amendment contains the following answers to questions received in regards to the Section D.16 pre-award inspection submittal package: QUESTION: It was my intention to send you a Verified Roster With documentation of verification for and copy of License EACH EMS Resource- listing EMPFs AEMTs and EMTs together in one document; A copy of our MT License and Permits; A copy of our Medical Directors Authorization; and a list of consumables with restocking fees, not multiple copies for EACH. ANSWER: Each solicitation will need a separate pre-award inspection documentation submittal package. Required submittals listed in Section D.16 that could be considered a company-wide submittal that apply to all resources being offered under a specific solicitation, such as the vendors Montana EMS provider license, medical directors letter, and list of consumables with restocking fees, can be submitted once per solicitation to cover a group of resources from that vendor under that particular solicitation. Example: If you are offering 10 ALS ambulances, you must submit a pre-award inspection submittal package containing a roster and Montana licenses of each individual EMT/AEMT/Paramedics being offered, individual Montana permits for each ambulance being offered, one (1) copy of the vendors Montana EMS service provider license, one (1) letter from a Montana Medical Director that covers all EMTs/AEMTs/Paramedics rostered under that vendor, and one (1) list of consumables with restocking fees that cover all 10 ALS ambulances. This will need to be done for each solicitation in accordance with Section D.16 of the solicitation. QUESTION: Will one submission for all 5 solicitations be sufficient, or should I send multiple copies? ANSWER: Each solicitation will need a separate pre-award inspection documentation submittal package submitted to the government by the vendor in accordance with Section D.16 of the solicitation. Please see the above answer for D.16 submission requirement clarifications.
